MR Number: 3-98-0049 Quad Cities 1 2 Date: 06/28/98 Cordova,Illinois Dockets: 50-254,50-265 BWR/GE-3,BWR/GE-3 Subject: Independent reactor scrams of Units 1 and 2 Discussion: At 5:11 p.m. on June 27, 1998, Unit 1 scrammed from approximately 100% power due to coincident half scrams when a level switch failed while a surveillance was being performed. A half scram signal was inserted in order to perform testing on average power range monitor instruments. During the test, a scram discharge volume level switch actuated, generating a scram signal on the other reactor protection system channel resulting in a full scram. The following reactor vessel level drop actuated containment isolation for groups 2 and 3 logic which was expected. The main steam isolation valves remained open and no safety relief valves opened. With minor exceptions, equipment performed as expected. The licensee corrected the level switch problem. At 2:23 a.m. CDT on June 28, 1998, Unit 2 scrammed during electrical storms. The cause appears to be output breaker trips resulting from damage to a transmission line miles from the plant. In addition, there was considerable lightning activity around the plant. The plant safety systems responded as expected without lifting safety relief valves, however some electrical anomalies occurred which may be lightning related. For example, the first out annunciator indicated that a manual scram was initiated. The licensee was investigating the trip further. Regional Action: Resident inspection staff responded to the site following each scram and are reviewing the licensee's cause investigations and observing the planned restart of Unit 1 today.
