This condition was | | brought to the attention of the operations shift manager, and upon | | evaluation and understanding, the breakers were immediately closed. | | | | Based on investigation so far, it appears that this condition existed since | | 12/16/98 when an electrical alignment was performed after maintenance on an | | electrical bus was completed. Maintenance of the electrical bus was started | | on 12/14/98. The alignment failed to restore the feeder breakers to the | | correct closed condition. Additionally, it appears that this condition | | rendered the standby shutdown system as described in technical | | specifications inoperable. This condition existed from 12/16/98 to | | 12/29/98. When the breakers were closed, the technical specification | | requirement for returning the standby shutdown system to operable status | | within 7 days was exceeded resulting in both units being in a unanalyzed | | condition. The valve is located on a | | secondary containment atmosphere sample line which connects to the | | post-accident sampling system. The licensee has verified that the valve is | | closed.
