The first | | test result was greater than the 3% tolerance band allowed from the | | setpoint, and the valve was declared inoperable. The valve setting did not | | require adjustment and has been retested within the 1% as-left tolerance | | required by Technical Specifications and returned to operable status. | | Although not confirmed, it is believed by the licensee that the condition | | that caused this valve to initially fail to lift within tolerances existed | | for a time greater than the 24 hours allowed by Technical Specification | | 3.7.1.1.a before a plant shutdown is required. While enroute to the | | onsite motor pool, the trailer came loose and dropped to the ground. No | | damage occurred to the tractor, the trailer, or the container. The driver | | notified the Tri-States dispatcher, who apparently notified Westinghouse of | | the incident. The message that Westinghouse received, however, was that the | | truck had overturned 17 miles from the site and separated from the trailer. | | Based on this erroneous information, Westinghouse notified the South | | Carolina Highway Patrol to have them verify the condition of the trailer. | | Westinghouse subsequently called the Highway Patrol to inform them that the | | original call was unnecessary since the truck had never left the site. | | | | At 1430, a Duke Power Company representative contacted the York County | | Emergency Management Office as a courtesy to make them aware of the Highway | | Patrol notification and the circumstances surrounding the incident. The BEO sent representatives to the | | C&C Alloy scrapyard in Oklahoma City to survey a crate of scrap metal which | | contained radioactive parts. Radiation levels of the scrap metal were | | reading twice as much as background levels. The metal had been cut up into | | small pieces and is believed to be magnesium thorium panels. No personnel | | had been exposed to respirable particles during the cutting process. The | | crate of material has been returned to TAFB for proper disposal. The BEO | | plans to establish by 01/15/99 a procedure to survey scrap metal before it | | leaves TAFB. The BEO plans to notify Oklahoma authorities and request that | | the Air Force Health Physics Branch (Brooks Air Force Base, San Antonio, TX) | | provide a radiation survey team if C&C Alloy requests that a survey of their | | scrapyard be performed. | | | | The USAF Radioisotope Committee was informed of this event at 1300 CST on | | 12/16/98. | +------------------------------------------------------------------------------+
