While attempting to perform the thyroid scan of the patient, | | technicians discovered that the patient had been administered Iodine-131 | | (I-131) instead of Tc-99m. The radiopharmaceutical delivered by Fort Smith | | Central Pharmacy had been mislabeled (the I-131 had been labeled as | | Pentechnetate). The State of Arkansas license number for Fort Smith Central | | Pharmacy is ARK-801-BP-08-98, and the Saint Edward Mercy Medical Center | | license number is ARK-335-BP-06-96.
