Review of the previous test performed (during | | Refueling Outage - R09) and past work history led to the conclusion that | | valve AC-MOV-797 may have been inoperable since the last time it was tested. | | Therefore, this valve may have been inoperable when containment integrity | | was required. | | | | Further investigation will continue to evaluate this issue. AC-MOV-797 will | | be repaired and properly retested prior to exiting Cold Shutdown. | | | | The NRC Resident Inspector was notified of this event notification by the | | Licensee. | +------------------------------------------------------------------------------+ +------------------------------------------------------------------------------+ |Power Reactor |Event Number: 35086 | +------------------------------------------------------------------------------+ +------------------------------------------------------------------------------+ | FACILITY: INDIAN POINT REGION: 1 |NOTIFICATION DATE: 11/29/1998| | UNIT: [2] [] [] STATE: NY |NOTIFICATION TIME: 23:47[EST]| | RXTYPE: [2] W-4-LP,[3] W-4-LP |EVENT DATE: 11/29/1998| +------------------------------------------------+EVENT TIME: 22:45[EST]| | NRC NOTIFIED BY: GOETCHIUS |LAST UPDATE DATE: 11/29/1998| | HQ OPS OFFICER: CHAUNCEY GOULD +-----------------------------+ +------------------------------------------------+PERSON ORGANIZATION | |EMERGENCY CLASS: N/A |NIEL DELLA GRECA R1 | |10 CFR SECTION: | | |AESF 50.72(b)(2)(ii) ESF ACTUATION | | |NLCO TECH SPEC LCO A/S | | | | | | | | +-----+----------+-------+--------+-----------------+--------+-----------------+ |UNIT |SCRAM CODE|RX CRIT|INIT PWR| INIT RX MODE |CURR PWR| CURR RX MODE | +-----+----------+-------+--------+-----------------+--------+-----------------+ |2 N Y 99 Power Operation |99 Power Operation | | | | | | | +------------------------------------------------------------------------------+ EVENT TEXT +------------------------------------------------------------------------------+ | ONE OF THREE EMERGENCY DIESEL GENERATORS IS OUT OF SERVICE PUTTING THE PLANT | | IN A 7 DAY LCO. | | | | DURING A MONTHLY SURVEILLANCE ON THE EDGs, THE "21" EDG FAILED THE | | SURVEILLANCE WHEN A FUEL OIL SUPPLY LINE FAILED.
