This system serves a small portion of the population | | within the 10 mile EPZ. All prompt notification system sirens have been | | tested and are available. Additionally, the paging system used to notify | | the Site Emergency Response Organization is out of service due to the same | | thunderstorm. The 58 | | inoperable sirens are divided between three counties as follows: 7 Berks | | County, 31 Montgomery County and 20 Chester County. The component | | cooling water (CCP) system has been leaking to the service water system at | | a rate of approximately 500 gallons per day. The CCP system contains | | hydrazine as a corrosion inhibitor and, as a result of interconnections | | with chilled water systems that contain freon, may also contain trace | | amounts of freon. The leakage from the CCP system would subsequently be | | released to the Long Island Sound via discharge #001C-5. An | | ambulance was summoned to transport the individual to a local hospital. The | | licensee was unable to perform a radiological survey of the individual's | | back and, therefore, the individual was considered to be potentially | | contaminated. The ambulance crew directed the Piedmont Medical Center to | | prepare for the impending arrival of a potentially contaminated individual. | | Licensee radiation protection personnel accompanied the individual to the | | hospital, and subsequently determined that the individual was not | | contaminated. | | | | The York County Emergency Preparedness Coordinator was monitoring the EMS | | frequencies on his scanner and overheard the discussion between the | | ambulance crew and the hospital. He then contacted the site directly to | | obtain additional information. The licensee has decided that this | | heightened interest in the event makes this report necessary. Power was lost | | to the 'B" reactor protection system (RPS) bus; however, a full scram | | signal was already present due to outage work. RPS power was restored from | | an alternate power supply. | | | | The following isolation signals and actuations occurred: | | | | Group 2* reactor water sample system | | Group 3* RHR system (Division 2) | | Group 4* RHR shutdown cooling and head spray | | Group 5* core spray system (Division 2) | | Group 6* HPCI system | | Group 7* HPCI system vacuum breakers | | Group 8* RCIC system | | Group 9* RCIC system vacuum breakers | | Group 11* RWCU outboard isolation | | Group 12* torus water management system | | Group 13 drywell sumps | | Group 14 drywell and suppression pool ventilation system | | Group 15* TIP system | | Group 16* nitrogen inerting system | | | | * These systems were properly removed from service before the event and no | | valve movement occurred. | | | | In addition, the reactor building HVAC system isolated and the standby gas | | treatment system started and maintained secondary containment. Control | | center HVAC shifted into recirculation mode. | | | | At the time of the event, the reactor cavity was flooded and Division 1 of | | RHR was being used for shutdown cooling. Shutdown cooling was not lost and | | reactor vessel water level was maintained greater than 20'6" above the | | vessel flange during the event. The licensee is investigating the cause of | | this event.
