UNIT 2 ENTERED A 24 HOUR LCO TO REACH COLD SHUTDOWN ON | | 8/7/94 WHEN IT WAS SUSPECTED THAT THE HPCI EXHAUST CHECK VALVES WERE | | DEGRADED DURING THE PERFORMANCE OF THE HPCI OPERABILITY SURVEILLANCE. POWER | | DECREASE WILL COMMENCE AT APPROXIMATELY 07:35 TODAY IF REPAIRS ARE NOT | | COMPLETED. | | | | THE EXHAUST LINE FOR HPCI HAS TWO CHECK VALVES IN SERIES PRIOR TO ENTERING | | THE TORUS. THE FIRST CHECK VALVE ENCOUNTERED LEAVING THE HPCI TURBINE IS | | 2-2301-45 AND THE SECOND VALVE IS A STOP CHECK VALVE (2-2301-74). THE 74 | | STOP CHECK VALVE HAS BEEN CLOSED. CURRENT PLANS ARE TO INSTALL A BLANK | | FLANGE AT THE 45 CHECK VALVE AND PERFORM ANOTHER LOCAL LEAK RATE TEST TO | | VERIFY INTEGRITY OF THE 74 STOP CHECK VALVE. THE VALVE WAS SUPPOSED TO CLOSE AUTOMATICALLY WHEN | | REACTOR VESSEL PRESSURE REACHED APPROXIMATELY 80 PSIG. THIS LOW REACTOR | | PRESSURE ISOLATION SIGNAL IS FOR PROTECTION OF THE HPCI TURBINE. THE VALVE | | COULD NOT BE CLOSED FROM THE CONTROL ROOM EITHER. THE VALVE HAD TO BE | | CLOSED USING THE MANUAL HAND WHEEL. AT THE TIME OF THE EVENT THE HPCI | | SYSTEM WAS INOPERABLE WITH THE INBOARD ISOLATION VALVE IN THE CLOSED | | POSITION. | | | | THE LICENSEE IS INVESTIGATING THE PROBLEM AND AT THIS TIME SUSPECTS AN | | ELECTRICAL MALFUNCTION. IT IS ASSUMED BY THE LICENSEE THAT THE OUTBOARD | | ISOLATION VALVE WOULD NOT HAVE CLOSED ON AN ACTUAL ESF SIGNAL. THE INBOARD | | ISOLATION VALVE ALSO CLOSES ON AN ESF SIGNAL, HOWEVER, BECAUSE THE VALVE | | WAS SHUT PRIOR TO RECEIVING THE SIGNAL THE LICENSEE IS NOT SURE IF IT WOULD | | HAVE OPERATED PROPERLY.
