THE AFFECTED ESF FILTRATION UNITS ARE THE | | CONTROLLED VENTILATION AREA SYSTEMS, SHIELD BUILDING VENTILATION SYSTEMS, | | CONTROL ROOM EMERGENCY FILTRATION SYSTEMS AND FUEL HANDLING BUILDING | | EMERGENCY FILTRATION SYSTEMS. THESE UNITS ARE DESIGNED TO START ON AN SIAS | | INCLUDING CONCURRENT LOSS OF OFFSITE POWER. AS THE CIRCUITRY EXISTS | | CURRENTLY, THE UNIT HEATERS WILL NOT RE-ENERGIZE AUTOMATICALLY AFTER LOSS | | OF POWER AND WILL CAUSE THE UNIT TO TRIP AFTER A 400 SECOND TIME DELAY ON | | LOW FILTER DIFFERENTIAL TEMPERATURE. A CHANGE IS BEING PLANNED TO RESET A | | TIME DELAY RELAY TO ALLOW THE HEATERS TO AUTOMATICALLY RE-ENERGIZE | | FOLLOWING A LOSS OF POWER. IN THE INTERIM, ADMINISTRATIVE CONTROLS HAVE | | BEEN ESTABLISHED TO MAINTAIN THE AFFECTED UNITS OPERABLE. AN OPERATIONS | | STANDING INSTRUCTION HAS BEEN PLACED IN EFFECT TO MANUALLY RESET THE | | HEATERS WITH THE HEATER PUSHBUTTON IN THE EVENT A LOSS OF POWER TO THE UNIT | | OCCURS." | | | | THE HEATER CONTROLS WERE REPLACED IN REFUELING OUTAGE 5, SEP 92. THE | | LICENSEE WAS UNABLE TO PROVIDE ANY FURTHER INFORMATION AS TO WHAT PROMPTED | | THE INVESTIGATION BUT DID NOTE THAT THIS PROBLEM WOULD NOT HAVE BEEN | | IDENTIFIED DURING SURVEILLANCE TESTING. BUS 33 WAS BEING SUPPLIED BY TRANSFORMER 31. THE | | OPERATOR ATTEMPTED TO CLOSE TRANSFORMER 32 TO BUS 33 BUT THE BREAKER | | TRIPPED FREE. LICENSEE WAS PERFORMING REMOTE OPERATION OF THE BREAKERS FROM | | THE CONTROL ROOM. THE OPERATOR CLOSED THE BREAKER TO TRANSFORMER 32 BUT HE | | DID NOT WAIT A SUFFICIENT AMOUNT OF TIME TO VERIFY THAT THE BREAKER HAD | | CLOSED BEFORE HE OPENED THE BREAKER TO TRANSFORMER 31 (OPERATOR THEN OPENED | | TRANSFORMER 31 TO BUS 33 NOT REALIZING THAT BUS 33 FEED FROM TRANSFORMER 32 | | DID NOT CLOSE). THE CAUSE OF THIS EVENT IS BEING INVESTIGATED. EMERGENCY | | DIESEL GENERATOR 2/3 CLOSED ONTO BUS 33-1. FLOYD) REPORTED A | | TRANSPORTATION EVENT IN I-80 NEAR DAVENPORT IOWA. | | | | AT ABOUT 1845 CDT ON 5/4/94, A PLACARDED FLAT BED TRUCK CARRYING TWO EMPTY | | POLY TANKS (10,500 GALLON CAPACITY) FROM THE DUANE ARNOLD PLANT TO BARNWELL | | S.C. HAD ONE OF THE TANKS STRIKE AN OVERHEAD BRIDGE ON I-80 (NEAR EXIT 290) | | NEAR DAVENPORT, IOWA. ONE TANK WAS KNOCKED OFF OF THE TRUCK AND ROLLED | | INTO THE MEDIAN. THE TANK IS LABELED AS LSA, HOWEVER, IT WAS EMPTY. THE | | TANK HAS ONLY FIXED INTERNAL CONTAMINATION, AND NO EXTERNAL CONTAMINATION | | HAS BEEN FOUND ON THE TANK OR THE GROUND IN THE VICINITY. THE TANK | | MEASURES 5 mr/hr ON CONTACT. THE TANK DID NOT RUPTURE AND DID NOT SUFFER | | ANY MAJOR STRUCTURAL DAMAGE. | | | | THE SHIPPER IS TRI-STATE MOTOR TRANSIT COMPANY, LOCATED IN JOPLIN, MO. AT | | THIS TIME, A CRANE IS ON THE SCENE TO RE-LOAD THE CONTAINER ON THE TRUCK. | | THE CONTAINERS WILL BE TRANSPORTED TO THE NEAREST STOP TONIGHT, AND WILL BE | | FURTHER EVALUATED IN THE MORNING IN DAYLIGHT. THE DAVENPORT FIRE | | DEPARTMENT AND HAZMAT TEAM RESPONDED TO THE SITE, AS WELL AS THE STATE | | POLICE. I-80 WAS NEVER CLOSED AS A RESULT, AND THERE ARE NO REPORTS OF | | MEDIA INTEREST AT THIS TIME. DUANE ARNOLD HAS SENT A SITE REPRESENTATIVE | | TO THE SCENE TO OFFER ASSISTANCE. | | | | DUANE ARNOLD MADE THIS NOTIFICATION DUE TO THE POTENTIAL OF OFF-SITE | | AGENCIES BEING NOTIFIED, AND HAS INFORMED THE RESIDENT INSPECTOR. | | | | MR. JIM LYNCH, OF REGION 3 CALLED THE OPS CENTER AT 2250 EDT TO INSURE WE | | WERE NOTIFIED. ACCORDING TO HIS INFORMATION (FROM THE IOWA DEPARTMENT OF | | HEALTH RESPONSE TEAM), APPROXIMATELY 200 GALLONS OF LIQUID WAS IN THE TANK. | | THE TANK PREVIOUSLY CONTAINED GLYCOL (WHEN USED AT DUANE ARNOLD), BUT WAS | | SHIPPED EMPTY. THE SOURCE OF THE 200 GALLONS OF LIQUID REMAINS UNKNOWN AT | | THIS TIME, BUT CONDENSATION WAS MENTIONED AS A POSSIBLE CONTRIBUTOR. HE | | ALSO CONFIRMED THAT BOTH DUANE ARNOLD AND THE IOWA DEPARTMENT OF HEALTH | | HAVE RESPONDED TO THE SCENE, AND THAT THE TANK WAS NOT STRUCTURALLY DAMAGED | | OR LEAKING. TO HIS KNOWLEDGE, ONLY SURFACE CONTAMINATION SURVEYS HAVE BEEN | | TAKEN OF THE AREA, AND NO WIPE TESTS AS OF YET.
