MR Number: 4-98-0057 South Texas 2 Date: 10/30/98 Wadsworth,Texas Resident Inspectors Dockets: 50-499 PWR/W-4-LP Subject: UNIT 2 STARTUP FOLLOWING REFUELING OUTAGE Discussion: On October 24, 1998, at 1:11 p.m. (CDT), the licensee completed their sixth refueling outage for Unit 2 (2RE06). Major activities associated with the outage include steam generator inspection, control rod drop testing, 2D reactor coolant pump motor replacement, vital battery replacement (1 bank), refurbishment of the #21 low-pressure turbine, and refueling. The outage lasted 21 days, 13 hours and 10 minutes, with an original schedule duration of 21 days. The unit reached 100 percent power on October 28, 1998. Regional Action: For information only. MR Number: 4-98-0058 Palo Verde 3 Date: 10/30/98 Wintersburg,Arizona Dockets: 50-530 PWR/CE80 Subject: PLANT RESTART FROM REFUEL OUTAGE NO. 7 Discussion: On September 19, 1998, the licensee shut down the Unit 3 reactor in order to begin a planned refueling outage (R7). On October 22, the licensee completed the outage related activities. Major work copleted during the outage include fuel replacement, reactor vessel 10 year inservice inspection, replacement of all 4 reactor coolant pump seals, 5 year service inspection of emergency diesel generator "A", Steam Generator inspections and a number of other minor maintenance activities. The outage lasted 36 days, 21 hours, and 23 minutes with an original scheduled duration of 36 days. On October 25 at 9:23 a.m. (CST), the licensee restored the unit to the grid.
