Date: 03/26/98 San Onofre 3 Resident Inspectors San Clemente,California Dockets: 50-362 PWR/CE Subject: RESTART OF UNIT 3 FROM CYCLE 9 MIDCYCLE OUTAGE Discussion: On March 26, 1998, at 12:02 p.m. PST, San Onofre Unit 3 connected to the grid following completion of a 19-day midcycle outage that included approximately 12 days in a midloop condition. The main purpose of the midcycle outage was to perform steam generator (SG) primary and secondary side (egg crate) inspections, pressurizer heater restoration, and reactor coolant system (RCS) inconel instrument nozzle replacement. As a result of the inspections, the licensee determined that 22 SG tubes required plugging based on eddy current inspection and an additional 27 tubes were plugged as a precautionary measure based on SG secondary side inspection. The licensee replaced 14 of 30 pressurizer heaters and abandoned one for replacement in the next refueling outage. The licensee performed weld repairs to 19 RCS inconel instrument nozzles. The licensee performed additional significant activities during the outage. The licensee installed one mechanical nozzle seal assembly on a pressurizer instrument nozzle and drilled the holes for future installation of two others. The licensee replaced all remaining 480V reversing line starters that had been previously determined to be susceptible to foreign material intrusion and degradation. The licensee replaced a Kerotest check valve in the charging system that was a similar design to valves in the charging system that had previously malfunctioned and that were replaced in the Cycle 9 refueling outage. The licensee replaced nine elbows on six moisture separator reheater drain lines as a result of flow accelerated corrosion. However, none of the lines were below minimum wall thickness.
