The gauge had been stolen from a temporary job site also located in Miami, Florida. The loss was reported to the NRC Operations Center on August 26, 1997. The gauge was intact inside its carrying case. However, neither the case nor the rod was locked. A preliminary wipe test performed by the Florida Bureau of Radiation Control indicated that no leakage of the source had occurred. The State has taken temporary possession of the gauge until the police department concludes their investigation after which the gauge will be returned to Arco Engineering and Testing Consultants (the owner). The State of Florida has closed its investigation of the incident and determined in September 1997, that the licensee did not violate any regulations of their license or State regulations. Decker (404)562-4721 _ REGION II MORNING REPORT PAGE 2 FEBRUARY 27, 1998 Licensee/Facility: Notification: An Agreement State Licensee MR Number: 2-98-0012 Raytech Date: 02/27/98 North Carolina Subject: THEFT OF SOURCES Discussion: The State of North Carolina, an agreement state, reported that one of their licensees, RAYTECH, had reported the theft of three check sources. The sources were manufactured in 1989-90 and had original activities of 188.4 microCuries of cesium-137, 270 microCuries of barium-133, and 5840 microCuries of cobalt-57 (not a NRC regulated radionuclide). The State conducted an inspection of RAYTECH on February 20, 1998 and the check sources are still missing. Regional Action: The State of North Carolina will be following up. Contact: T. MR Number: 4-98-0009 Comanche Peak 2 Date: 02/26/98 Glen Rose,Texas Resident Inspector Dockets: 50-446 PWR/W-4-LP Subject: UNIT 2 TURBINE RUNBACKS DURING LIGHTNING STORM Discussion: This morning report provides additional information and clarification to the February 26, 1998, NRC daily status report. At approximately 7:41 p.m. (CST) and 8:25 p.m. (CST) on February 25, 1998, the Comanche Peak Steam Electric Station, Unit 2, experienced two runbacks during an electrical storm. While there were no losses of offsite power sources or damage to plant equipment from the strikes, reflections from the grid were sensed by the main generator control circuitry, thereby causing the runbacks. The first runback left the plant operating at approximately 94 percent reactor power; within the next 15 minutes, the licensee manually reduced reactor power to below 90 percent power, as required by plant Technical Specification 3.2.1, "Axial Flux Difference." The second runback left the plant operating at approximately 57 percent reactor power. During the second runback, low pressure feedwater heating and the extraction steam to high pressure feedwater Heater 1-3 were lost. Because of accumulated axial flux difference penalty minutes, the licensee manually reduced reactor power less than 50 percent. Nuclear instrument setpoints were appropriately reduced to 55 percent within the next 4 hours. Troubleshooting to identify the specific cause of the runbacks continues. Thus far, the licensee has found that the runback circuitry and its inputs were working properly. The plant will be restored to 100 percent power on February 27, 1998. This is the fifth lightning related plant transient since 1995. The licensee is reviewing the corrective actions that have previously been implemented to determine if there is a continuing vulnerability at the site.
