MR Number: 1-98-0012 Limerick 1 2 Date: 02/25/98 Philadelphia,Pennsylvania Dockets: 50-352,50-353 BWR/GE-4,BWR/GE-4 Subject: INDUSTRIAL ACCIDENT AT LIMERICK Discussion: On February 23, 1998, an employee of a subcontractor to the Overhead Crane Company was injured while performing calibrations of a hoist limit switch on top of a turbine building crane. The individual was wearing a safety harness with an attached lanyard, which was not tied-off since the individual was working on a platform with railings. The lanyard became entangled in a rotating portion of the crane pulling the individual into the machinery causing him to receive several serious injuries. The individual was partially frisked during the removal and later confirmed to be not contaminated. The licensee secured the scene of the accident to perform a formal detailed investigation of the event on February 24, 1998. The investigation will involve PECO Energy, Overhead Crane, and representatives to the parent company of Overhead Crane. The resident staff is continuing to monitor PECO Energy's continuing assessments and corrective actions. MR Number: 4-98-0008 Palo Verde 1 Date: 02/23/98 Wintersburg,Arizona Licensee notified SRI Dockets: 50-528 PWR/CE80 Subject: UNIT 1 RESTART FOLLOWING UNPLANNED SHUTDOWN Reportable Event Number: 33770 Discussion: On February 22, 1998, at 10:22 p.m. (MST), while operating at 100 percent power, Palo Verde Unit 1 automatically tripped on low level in Steam Generator 2. The low level condition resulted from unexpected closure of the Steam Generator 2 economizer control valve. At the time, there were no activities in progress that would affect operation of the economizer control valve. As expected, the auxiliary feedwater actuation system initiated in response to the low steam generator water level. The trip was uncomplicated, with major safety and balance of plant systems responding as expected. The cause of the trip was determined to be a faulty card in the steam generator level control circuitry. The card was replaced and tested satisfactorily. Unit 1 restart commenced at 10:56 p.m. on February 23, 1998, with withdrawal of the shutdown and part length rods. The Unit entered Mode 2 at 2:50 a.m. and went critical at 3:34 a.m. on February 24, 1998. The licensee estimates that the Unit will be connected to the grid in the afternoon on February 24, 1998. Power ascension testing will be conducted at 20%, 40%, 75%, and 90% power plateaus. Regional Action: The resident inspectors are monitoring the licensee's power ascension.
