MR Number: 1-98-0034 Haddam Neck 1 Date: 06/22/98 Hartford,Connecticut Dockets: 50-213 PWR/W-4-LP Subject: INADVERTENT RELEASE OF RADIOACTIVE LIQUID Discussion: With the plant currently shutdown for decommissioning and the reactor defueled, the licensee had obtained an NPDES permit to release the A' and B' waste test tanks into the Connecticut River on 6/20/98. During release of the B' tank, a cross-connect line between the two tanks was inadvertently opened, which resulted in approximately 800 gallons of radioactive liquid (total activity of 2252 microcuries, excluding tritium and noble gases) to be inadvertently released from the A' tank, concurrent with the ongoing B' tank release (10,000 gallons). The open valve was discovered during a walk-down of the valve-line up, which was required in response to the liquid radioactive waste monitor set-point alarm. No release limits were exceeded. On 6/22/98, licensee post event review of the Adverse Condition Report, which was filed by the shift manager for the above evolution, determined that the inadvertent discharge had met the criteria for an Unusual Event, because the release, although planned, had inadvertently occurred and was greater than 1000 microcuries. Ongoing activities in the area of the tank may have caused a worker to have bumped open the ball-valve on the cross-connect. The licensee notified the NRC on 6/22/98 at 1148 (EDT) per 10CFR 50.72 (b)(2)(vi), however, the report had indicated erroneously that the total activity was 1000 times greater than the actual event. Upon NRC follow-up, the licensee provided a corrected and updated report on 6/22/98 at 1737 (EDT). Initial corrective actions by the licensee were to confirm the valve line up for all liquid waste discharges, and to defer all discharges from the two waste test tanks until it is confirmed that valves susceptible to bumping or unintentional manipulation have been adequately protected. The licensee committed to conduct a root cause investigation with a written report due in 30 days. The State was notified by the licensee. At this time, no press release is planned. Regional Action: A Region I Health Physicist is onsite reviewing the licensee's response and follow-up to the inadvertent release and the ensuing communication errors. The Senior Resident Inspector will continue to review the licensee's activities over the next few weeks. The NRC has also been in communication with the State on this matter. MR Number: 2-98-0032 Robinson 2 Date: 06/23/98 Hartsville,South Carolina Dockets: 50-261 PWR/W-3-LP Subject: DEMONSTRATION AT THE ROBINSON SITE Discussion: On June 22, 1998, at approximately 1:10pm, the Robinson Plant Site Security Manager, S. Young, informed an NRC regional security inspector that there had been a demonstration outside of the plant from 10:10am to 11:20am, June 22, 1998. The demonstrators called themselves "CAFE," Citizens Alliance for Fair Employment. The demonstration was against purported CP&L hiring practices concerning African-Americans. There were approximately 38 demonstrators: 10 adults, and the rest children between the ages of 4 and 14. The demonstrators did not enter CP&L property, nor were there any threats or acts of violence. Local TV media was at the demonstration and local law enforcement was present. Contact: W.
