Date: 02/23/98 San Onofre 2 SRI was notified San Clemente,California Dockets: 50-361 PWR/CE Subject: RESTART FROM CYCLE 9 MIDCYCLE OUTAGE Discussion: On February 22, 1998, San Onofre Unit 2 restarted following completion of a 30-day midcycle outage. During the outage, the reactor coolant system (RCS) was operated in midloop conditions for approximately 22 days to facilitate steam generator (SG) inspections and RCS loop nozzle repairs. The purpose of the midcycle outage was to inspect SG tubes, based on primary water stress corrosion cracking (PWSCC) observed at internal support (egg crate) intersections during the last refueling outage. During the midcycle outage, the licensee performed a bobbin inspection of 100 percent of all in-service tubes and plus point inspections of selected tube portions. As a result of the inspections, the licensee determined that 159 tubes required plugging in SG 2E088 and 112 tubes required plugging in SG 2E089. Of these 271 tubes, only 12 required plugging due to PWSCC at egg crate locations. One hundred seventy-one tubes were plugged because of freespan axial indications in the lower part of hot leg tubes, and 50 tubes were plugged because of outside diameter stress corrosion cracking at egg crate locations. The results of in situ pressure testing of 13 tubes with bounding flaws were satisfactory. The licensee performed weld repairs of 24 RCS instrument nozzles (12 hot leg and 12 cold leg) that were susceptible to PWSCC of their Inconel-600 material. The licensee installed mechanical nozzle seal assemblies (MNSAs) on two pressurizer instrument nozzles, also due to the potential cracking of Inconel-600. Due to leakage indications identified by the licensee at the beginning of the outage, MNSAs were also installed on four other instrument nozzles (two on the SG 2E089 lower bowl and two on the SG 2E088 RCS hot leg). Three of the nozzles that were weld-repaired were also identified as having indications of leakage. In all cases, the leaking nozzles showed only minute indications of leakage. While replacing a reversing line starter for the motor-operated valve actuator for the Train A containment emergency sump outlet valve, the licensee identified that the mechanical interlock was stuck in the "close" position, which would have prevented the valve from being opened. The licensee subsequently identified and replaced all remaining susceptible reversing line starters in Unit 2 and inspected those in Unit 3.
