Skeen NEW ISSUE. The Cooper licensee reports failure of an ASCO solenoid valve that was installed in the reactor building ventilation system. The licensee attributes the most likely cause for failure to brass strands discovered in the valve. Date: 05/21/98 Subject: Peebles Electric L.L.C. Discussion: As a result of correspondence received from Peebles Electric L.L.C., the NRC has become aware that this vendor will no longer be available for repair/spare parts, field service or technical assistance and will soon dispose of all records and design basis information in their possession. Peebles Electric has supplied static exciter-voltage regulators which have been used with emergency diesel generators. All reactor licensees known to have used their product have been notified by Peebles Electric of this intention and many have initiated steps to purchase the documentation that Peebles Electric believes is necessary for continued long term use of their product. All actions with Peebles Electric must be initiated by June 1, 1998. Contact: Jerry Carter, NRR/DRPM/PECB (301) 415-1153 Email: tjc@nrc.gov _ REGION IV MORNING REPORT PAGE 3 MAY 21, 1998 Licensee/Facility: Notification: Omaha Public Power District MR Number: 4-98-0030 Ft Calhoun 1 Date: 05/20/98 Fort Calhoun,Nebraska SRI via telephone Dockets: 50-285 PWR/CE Subject: LOSS OF OFFSITE POWER Reportable Event Number: 34267 Discussion: On May 20, 1998, at approximately 2:11 p.m. (CDT), a loss of offsite power occurred as a result of a fault on Transformer T1A-3, a 161 kV to 4 kV transformer which supplies power to one of two vital electrical buses. The 161 kV supply feeder breaker upstream of Transformers T1A-3 and T1A-4 (which was supplying the second vital electrical bus) tripped, resulting in a complete loss of offsite power to the facility. The emergency diesel generators started and loaded onto the vital electrical buses as designed. Shutdown cooling was lost for several seconds as the diesel generators started and restored power to Buses 1A3 and 1A4. The plant had previously been shutdown and was in the 50th day of a refueling outage with the new core loaded, the reactor vessel head installed, the reactor coolant system filled, and shutdown cooling in service. A Notification of Unusual Event was declared at 2:31 p.m. (CDT). Offsite power was restored to vital Buses 1A3 and 1A4 by backfeeding through the main transformer with a separate 345 kV offsite power supply at approximately 3:50 p.m., and the Notification of Unusual Event was terminated at 3:57 p.m. Inspection of Transformer T1A-3 by the licensee has not, to date, identified any damage to the transformer other than an arc track down the Phase C bushing and between the Phase B and C insulators on the primary side. No cause for the arc had been determined, and the licensee is making preparations for restoration of 161 kV power. Regional Action: A special inspection was initiated on May 21, 1998. Contact: W. D. MR Number: 4-98-0031 Arkansas Nuclear 2 Date: 05/20/98 Russelville,Arkansas Senior Resident Inspector Dockets: 50-368 PWR/CE Subject: UNPLANNED REACTOR SHUTDOWN Reportable Event Number: 34265 Discussion: On May 20 at 6:54 a.m. Arkansas Nuclear One, Unit 2, began a plant shutdown due to a condenser tube leak. The reactor was taken off line at 8:07 a.m. While operators were placing the auxiliary feedwater system in service, steam generator levels lowered below the setpoint for automatic emergency feedwater initiation. Both trains of emergency feedwater initiated, restoring steam generator water level. Operators established auxiliary feedwater and secured the emergency feedwater pumps. The duration of the event was 9 minutes. Postshutdown investigations found one significant condenser tube failure and two tubes with minor leakage. Repairs to the affected components have been completed. The licensee is restoring proper steam generator chemistry in preparation for returning the unit to service.
