Headquarters Daily Report JANUARY 20, 1998 *************************************************************************** REPORT NEGATIVE NO INPUT ATTACHED INPUT RECEIVED RECEIVED HEADQUARTERS û REGION I û REGION II û REGION III û REGION IV û PRIORITY ATTENTION REQUIRED MORNING REPORT - HEADQUARTERS JANUARY 20, 1998 Licensee/Facility: Notification: MR Number: H-98-0005 Beaver Valley 1,2 Date: 01/20/98 Subject: Westinghouse 7300 Series Comparator Cards Discussion: On December 19, 1997, the licensee for the Beaver Valley Power Station (BVPS) identifed a lack of electrical separation condition in the Unit 2 emergency diesel generator (EDG) rooms' temperature control in the "A" and "B" Secondary Process Racks. The licensee determined that the cause was a design error during original plant construction (isolators were missing due to their deletion from design drawings). The isolators were installed and the licensee performed a review to determine what other systems may have similar problems. On December 29, 1997, with Unit 1 in hot standby and Unit 2 in cold shutdown, BVPS reported a nonconforming condition related to the Westinghouse 7300 Series comparator cards installed in the Unit 2 secondary process racks. Improper isolation of class 1E and non-class 1E devices (comparator, computer input, indicators, and channel test switch) was identified. Class 1E power was supplied to a non-class 1E 7300 comparator card without a qualified class 1E isolation device. The lack of separation was also due to a design error during original plant construction. The proper electrical isolation/separation was subsequently established by modifiying the 7300 card connections and internal 7300 panel wiring. Additionally, BVPS reported that various 7300 cards have a capacitor connected across the input to the card power supply. With this arrangement the card power supply fuse will not clear a capacitor short circuit, tripping the primary and secondary 7300 panel power supply breakers and consequently disabling a 7300 protection train. BVPS and the Westinghouse Owners Group continue to investigate this issue. BVPS investigated the applicability of these issues to Unit 1, focusing on the 7300 series equipment and the 7100 Series Secondary Process Racks. No design errors were identified although classification discrepancies between some equipment and the Master Equipment List were identified. These findings are being compiled in an information notice to alert licensees to these generic problems with providing the adequate respiratory protection during emergencies. Dacimo, currently LaSalle Plant General Manager, is replacing Mr. William Subalusky as Site Vice President. Mr. Subalusky will be heading up a key strategic initiative, improvement of nuclear training, for the Nuclear Generation Group. On January 19, 1998, Mr. Fred Dacimo announced the following management changes: Tim O'Connor, Operations Manager at Zion Station, will assume the position of LaSalle Station Manager and report to the Site Vice President. Steve Smith, LaSalle Plant Manager, is being reassigned as the Restart Manager and will report to the Station Manager. Jimmy Baily, LaSalle Restart Manager, is being reassigned to a newly created position, Site Business Manager, and will report to the Site Vice President.
