Hake Associates Date: 06/19/98 Memphis,Tennessee Subject: MINOR TRANSPORTATION ACCIDENT INVOLVING RADIOACTIVE MATERIAL Discussion: At 5:00 p.m., EDT, on June 17, 1998, a tractor trailer owned by Frank W. Hake Associates, Memphis, TN, was involved in a minor traffic accident. The truck was carrying scaffolding contaminated with low-levels of fixed mixed fission products. The shipment was identified as containing Limited Quantities of radioactive material. Several 1.5 inch metal rods penetrated the trailers rear doors about six inches. The metal rods were relocated, the holes covered and the truck was moved to a nearby truck stop. On June 18, 1998, the State of Tennessee Division of Radiological Health authorized the truck to proceed to the final destination, American Ecology Recycling Center in Oak Ridge, TN.
