The vendor reports that in June 1998 the McGuire licensee discovered a broken valve spring on one of its emergency diesel generators and subsequently attributed the break to high-cycle fatigue cracking under torsional loading. This was a result of a significant decarburized surface layer and that the spring was manufactured from a lower-strength material than specified. Four of the 132 springs furnished to the McGuire licensee for its Nordberg Model FS131HSC emergency diesel generator were manufactured from material that did not meet the specifications. The Millstone Unit 1 licensee reports an improper fit of the bearings in a tachometer transducer used on a gas turbine generator that is dedicated for emergency power for vital ac buses during an event. The transducer provides an overspeed signal to the governor. It was supplied by Eitel Manufacturing, Capistrano Beach CA. The licensee determined that a finger spring washer was installed in the incorrect orientation, resulting in the improper fit of the bearings. The Events Assessment Branch is not aware of any other safety-related applications of this product.
