Information Notice 98-18, "RECENT CONTAMINATION INCIDENCES RESULTING FROM
FAILURE TO PERFORM ADEQUATE SURVEYS," was issued on May 13, 1998.        
                                                                         
This notice was issued to all medical licensees to alert them to recent  
incidents of radiopharmacies receiving contaminated packages from client 
facilities.  In each case, the clients failed to resurvey shipping       
packages after a spill occurred near the packages.
