MR Number: 3-98-0034 Fermi 2 Date: 05/11/98 Newport,Michigan via FERMI 2 RPM Dockets: 50-341 BWR/GE-4 Subject: IRRADIATED HARDWARE IN AN EMPTY TYPE B SHIPPING CASK RECEIVED FROM BARNWELL Discussion: During an inspection of a Type B shipping cask to be used for transporting irradiated hardware, Fermi 2 staff discovered a 4" piece of a boron tube in the cask. Initial surveys indicated gamma dose rates up to 40R/h on contact. There was no discernable dose rate outside the cask, and no radiation exposures occurred. Chem- Nuclear at Barnwell was contacted by the licensee, and the Chem-Nuclear RPM indicated that visual inspections and radiation surveys at their facility were inadequate to ensure the cask was "empty" prior to its shipment to Fermi 2. Barnwell staff are scheduled to meet with State of South Carolina officials on Thursday, May 14, 1998, to discuss this event. Barnwell indicated that they received the cask loaded with irradiated hardware from LaSalle for land burial of the material. LaSalle representatives have visited the Barnwell facility as a result. Fermi 2 staff have planned to conduct further characterization through radiological surveys and pictures. Region 3 and Chem-Nuclear will be informed of the findings. Current plans are to ship this irradiated hardware to Barnwell either singly or as part of a larger Fermi 2 shipment. Regional Action: Region 3 staff have discussed the initial findings with Fermi 2 RP personnel, and the Region 2 office was contacted. Region 3 will continue to follow up the disposition of this material with Fermi 2 staff. MR Number: 3-98-0035 Kewaunee 1 Date: 05/13/98 Kewaunee,Wisconsin SRI Dockets: 50-305 PWR/W-2-LP Subject: KEWAUNEE MANAGEMENT CHANGES ANNOUNCED Discussion: On May 12, 1998, the licensee announced that Mr. K. Weinhauer will become the new Kewaunee Plant Manager, tentatively effective June 1, 1998. Mr. K. Weinhauer succeeds Mr. C. Schrock, who is being promoted to Vice President - Energy Supply, a non- nuclear position at the licensee's corporate headquarters in Green Bay, WI. Mr. Weinhauer's previous position was that of Assistant Plant Manager - Operations. Mr. Schrock will remain at the Kewaunee site until early June to provide for a transition period. Additionally, Mr. M. Marchi, currently Nuclear Business Group Manager, has been assigned to a newly created position of Site Vice President. Discussion: On May 11, 1998, the licensee announced that Mr. Vito Kaminskas will assume the role of Regulatory Services and Licensing Manager. Mr. Kaminskas most recently was employed by the Florida Power and Light Company. Mr. Kaminskas replaces Mr. Douglas Johnson who will assume a position on the Company's strategic planning group.
