The notice was issued to all medical licensees to alert them to 2 reported incidents during the conduct of intravascular brachytherapy procedures using the Novoste Beta-Cath system. Both incidents involved the failure of the high dose rate (HDR) sources to return to the device storage safe at the conclusion of the treatments. The loose connections resulted from incorrect application of heat sink clamp hardware for silicon controlled rectifiers and power diodes. MR Number: 1-98-0014 Maine Yankee 1 Date: 03/12/98 Wiscasset,Maine TELEPHONE FRM SRI Dockets: 50-309 PWR/CE Subject: COMPLETION OF REACTOR SYSTEM CHEMICAL DECONTAMINATION Discussion: On Friday March 6, 1998, the Maine Yankee Atomic Power Station completed the second phase of a two-phase chemical decontamination of portions of the reactor system. The process was initiated on February 14th and was completed within the scheduled time frame with no major problems encountered. Sluicing and de-watering resins and removal of the sixth and final high-integrity container of spent resins was completed March 9th. Although the process removed less total activity than originally estimated, decontamination factors (DF's) achieved at specific piping locations met expectations (measured DF's from approximately 8 to 225 were achieved). General area exposure rates were also significantly reduced with some areas reduced by as much as a factor of nine. Removal of temporary piping and equipment is in progress.
