MR Number: 2-98-0023 Vogtle 2 Date: 05/11/98 Waynesboro,Georgia Dockets: 50-425 PWR/W-4-LP Subject: REACTOR/TURBINE TRIP DUE TO LIGHTNING STRIKE Discussion: On May 8, 1998, at 4:01 a.m., Vogtle Unit 2 experienced a reactor trip from 100 percent power. The reactor trip was caused by a main generator/turbine trip due to a lightning strike near the switchyard to one of the two 500 KV transmission lines which supplies the Unit 2 Main Stepup Transformer. As a result of the strike, the breakers connecting the Main Stepup transformer to the 500 KV supply line opened. During the voltage transient, the Phase A Main Stepup Transformer suffered an electrical fault resulting in a Turbine/Generator trip followed by reactor trip. Subsequent investigations by the licensee revealed that the Phase A Main Stepup Transformer suffered both electrical and mechanical damage. Actions were initiated to tie in a spare phase transformer. Inspections of the other transformer phases found no damage. The reactor was taken critical May 9, at 3:58 a.m., in anticipation of completing the tie in of the spare phase transformer. On May 10, after further discussions with General Electric Company, concerns were raised regarding potential damage to the Generator as a result of the voltage transient experienced during the lightning strike. At 3:21 p.m., the unit was shutdown to Mode 3 in order to perform additional inspections of the generator. The plant is currently in Mode 3 awaiting completion of these inspections. Regional Action: Upon notification of the event, the senior resident inspector responded to the site. The resident staff will followup up on the licensee's event recovery activities, inspections of the main stepup transformer, and preparations for unit restart. Contact: R. M. Hale, Radiological Manager, has been named as the Senior Manager of Safety/Site Support, on an interim basis. Mr. Hale is taking the position vacated by Paul J. Caudill, who is currently the Acting Senior Engineering Manager. Mr. Hale will perform the duties of both the Radiological Manager and Senior Manager of Safety/Site Support until a permanent replacement for the Senior Engineering Manager is identified. MR Number: 4-98-0027 Arkansas Nuclear 1 Date: 05/10/98 Russelville,Arkansas Senior Resident Inspector Dockets: 50-313 PWR/B&W-L-LP Subject: PLANT STARTUP FROM REFUELING OUTAGE Discussion: On May 10, 1998, Arkansas Nuclear One, Unit 1, connected the main generator to the grid to complete Refueling Outage 1R14. The refueling outage began on March 28, 1998. Major activities accomplished during the refueling outage included refueling of the reactor; inspections of both steam generators; and replacement of the main condenser, a reactor coolant pump motor, the reactor building purge valves, one of two low pressure turbines, the emergency batteries, and control rod drive mechanism power cables.
