The water hammer resulted from an unintended startup of all four fire water pumps following a fire system "pre-action" which filled the normally dry sprinkler headers in an emergency diesel generator room. MR Number: 1-98-0036 Haddam Neck 1 Date: 07/07/98 Hartford,Connecticut Dockets: 50-213 PWR/W-4-LP Subject: APPARENT TAGOUT ERROR AND VALVE MISALIGNMENT Discussion: With the plant permanently shutdown for decommissioning and the reactor defueled, an inadvertent spill of approximately 350 gallons occurred in the spent fuel pool building. The spill happened during work to complete the spent fuel pool cooling/make-up system modifications and resulted in spraying water on a few workers. Demineralized water with no measurable radioactivity was being added to another system and was thought to be isolated from the spent fuel pool make-up/cooling system. However, the water was released because a valve was left out of position in the primary grade water system (a valve that was required to be closed per a safety tag was actually open). Water sprayed from pipes that were cut during a tie-in for the sytem modification in the spent fuel pool make-up/cooling system. Work on the job was stopped. The licensee monitored personnel and did not identify any radiological contamination. A small amount of water leaked outside the building, but no radioactive contamination was identifed from the spill outside the building. The licensee's staff was in the process of cleaning up the spilled water. An event review team was being assembled by the licensee to investigate the cause of the event. Licensee management stopped all work on the site except tasks important to safety or regulatory compliance. In addition, the licensee will conduct procedure reviews, perform personnel training, and restate management expectations to the staff. No press release was planned. Regional Action: The SRI will continue to monitor the licensee's investigation and perform independent reviews of the event. The State of Connecticut has been notified.
