Headquarters Daily Report JULY 06, 1998 *************************************************************************** REPORT NEGATIVE NO INPUT ATTACHED INPUT RECEIVED RECEIVED HEADQUARTERS û REGION I û REGION II û REGION III û REGION IV û PRIORITY ATTENTION REQUIRED MORNING REPORT - REGION IV JULY 6, 1998 Licensee/Facility: Notification: Washington Public Power Supply System MR Number: 4-98-0042 Washington Nuclear 2 Date: 07/06/98 Richland,Washington Resident Inspector Dockets: 50-397 BWR/GE-5 Subject: Plant Restart Following Forced Outage and Flooding Event Discussion: WNP2 returned to power operation and synchronized to the grid at 11:32 p.m. (PDT) on July 4, 1998, following a forced outage and flooding event. WNP2 performed a manual plant shutdown on June 15, 1998, as a result of a detached and stuck traversing incore probe. At the time of the shutdown, the facility had been operating at 38 percent power and was performing power ascension testing following an extended refueling outage which began on April 18, 1998. Major work items during the refueling outage included replacement of the emergency core cooling system suction strainers, core shroud inspections, and replacement of the local power range monitors. On June 15, with the facility in Mode 4, a water hammer occurred in the fire protection system which ruptured a fire main valve. Subsequent flooding resulted in the submersion of one residual heat removal pump. The licensee declared a notice of unusual event due to the flooding, and the NRC entered the monitoring phase of the normal mode of operations. An NRC augmented inspection team was dispatched to the site on June 18 to review the circumstances surrounding the flooding event. The augmented inspection team will conduct a public exit of their findings on July 8. A management meeting between the NRC and the licensee was held in the Region IV office on July 2. As committed by the licensee, the meeting was held to discuss issues associated with the broken fire main and flooding event at WNP2, along with the licensee's planned approach to restart. Subsequent to the management meeting, the licensee completed fire protection system testing that demonstrated that the root cause for the broken fire main had been eliminated. All short-term corrective actions were completed and the licensee commenced restart of the plant. Regional Action: The resident inspectors will follow the licensee's actions to return to full power. Region IV monitored the licensee's actions during the forced outage and flooding event, and dispatched inspectors to augment the resident staff during and after the event.
