These included improper installation of a reactor protection system (RPS) power supply that resulted in failure of the RPS Subchannel A2 scram discharge volume high level trip to actuate in response to a high level (Channels A1, B1, and B2 functioned properly); and small amounts of coal tar lining that flaked off from the interior of service water piping. In addition, engineers discovered fused disconnects on the 125 Vdc battery buses with insufficient rated current interruption capacity (although subsequent testing demonstrated that the fuses would have performed their intended function) and an emergency diesel generator output breaker that closed unexpectedly when racked in, resulting in motoring the diesel for a few seconds (inspections revealed no damage to the generator). Also, workers simultaneously opened two shutdown cooling drain valves at different elevations, causing much more water than expected (greater than 500 gallons total) to drain into the northwest quadrant room of the reactor building. As a result, the residual heat removal pump motors were sprayed down, necessitating corrective action to ensure that no damage occurred. The licensee has pursued and implemented corrective actions for each of these problems. As of 7 a.m. on November 6, the licensee anticipated restart from the outage on November 21.
