The incident involved victims of carbon monoxide poisoning (vice ammonia vapors) on a barge operating in the Chesapeake Bay. No press release was made by BGE. Mr. Polston is scheduled to leave his position in mid-July. Mr. Pulley has served as the Paducah Enrichment Plant Manager since 1995. Prior to assuming those duties, Mr. Pulley served as the Deputy Plant Manger from 1990 to 1995, and held numerous other management positions at the plant since his initial employment in 1966. Regional Action: Information Only Contact: PAT HILAND (630)829-9603 _ REGION IV MORNING REPORT PAGE 3 JUNE 5, 1998 Licensee/Facility: Notification: Omaha Public Power District MR Number: 4-98-0036 Ft Calhoun 1 Date: 06/05/98 Fort Calhoun,Nebraska Telephone call from SRI Dockets: 50-285 PWR/CE Subject: END OF REFUELING OUTAGE 17 Discussion: At 11:12 p.m. (CDT) on June 4, 1998, the licensee closed the main generator output breakers to end Refueling Outage 17. The outage duration was 65 days. Major work completed during the outage included the following: - Steam generator eddy current testing and tube plugging - Installation of an additional lube oil collection system around the reactor coolant pumps - Replacement of the 6th stage extraction steam piping - Modifications to the shutdown cooling isolation valves - A modification to the steam-driven auxiliary feedwater pump The outage, which was originally planned for 42 days, was extended as a result of problems encountered during installation of the modifications to the shutdown cooling isolation valves and maintenance on the steam-driven auxiliary feedwater pump. The reactor was made critical at 10:10 p.m. on June 3. Low power physics testing was completed, and Mode 1 was entered at 12:46 p.m. on June 4.
