MR Number: 4-97-0074 River Bend 1 Date: 09/30/97 St Francisville,Louisiana Resident Inspector Dockets: 50-458 BWR/GE-6 Subject: CRACKED CYLINDER VALVE ADJUSTING SCREW ASSEMBLY PADS Reportable Event Number: 33004 Discussion: On Sunday September 28, 1997, the licensee reported (Event Number 33004) the failure of some Division 1 and 2 emergency diesel generator (EDG) cylinder valve adjusting screw assembly pads. The licensee had declared both Division 1 and 2 EDGs inoperable on September 26. The failures occurred on TransAmerica Delavall, Inc. diesels. On Monday September 29, 1997, a telephone call was held with the licensee, the Office of Nuclear Reactor Regulation, and Region IV to obtain additional information. Licensee records indicated that the cracked valve adjusting screw assembly swivel pads (VASASP, P/N 1A-7840) were from Lot VV64, which was supplied by Cooper Enterprise in Grove City, PA. Licensee metallurgical testing showed that the VASASPs contained material of incorrect hardness. The licensee procured new VASASPs from the vendor of the correct material composition (Lot PP56). The licensee installed these VASASPs in the Division 1 EDG. The Division 1 EDG was tested and returned to operable status at 3:41 p.m. (CDT) on September 27, 1997. Replacement of the Division 2 EDG VASASPs is in progress. The licensee is in the process of conducting a root cause analysis of this event. Regional Action: Resident inspector followup. Contact: E.
