Examination of the bag identified a wrist badge from St. Elizabeth Health Center in Youngstown, Ohio. Region III contacted St. Elizabeth Health Center, an NRC licensee (License No. 34-01131-01), and determined the patient identified by the wrist badge had been treated with 115 millicuries (4.2GBq) of iodine-131 on 12/10/97. The patient had been released on 12/11/97 in accordance with 10 CFR 35.75. The licensee and the State of Ohio believe the radioactive material was disposed of at the patient's residence. This belief was reenforced by the landfill's assertion that all trash pick up in the container where the bag was located was from residential areas and not from a hospital. The Mahoning County Landfill is holding the material in a 55 gallon drum until decay has reduced radiation levels to background after which the material will be disposed of in the landfill. MR Number: 4-97-0094 Waterford 3 Date: 12/26/97 Killona,Louisiana Resident Inspector Dockets: 50-382 PWR/CE Subject: SPENT RESIN SPILL Discussion: On December 26, 1997, at 11:15 a.m. (CST), spent resin spilled into the spent resin pump room during recycling of the spent resin tank. Upon investigating, personnel noted a resin slurry moving toward the doorway into the room. The licensee took prompt action to stop the spill and to limit its spread. Subsequent surveys indicated that the radiation dose rate was 20 rem per hour 3 feet from the edge of the spilled resin and 25 mrem per hour at the doorway into the room. The resin spill is contained in the room and the licensee estimates that 50 percent of the floor of the room is covered with spilled resin at a depth of 4-6 inches. The spent resin is now dry, and no airborne radioactivity has resulted from the spill. The room is maintained at a negative pressure with air flow into the room, and ventilation for the room is filtered through the normal auxiliary building plant stack. The total exposure to personnel to date related to this event has been 120 mrem, which included personnel trying to prevent the resin slurry from reaching the doorway and a radiation protection technician who received 70 mrem while photographing and surveying the spill. The licensee has initiated plans to use a robot to clean up the spilled resin to minimize personnel exposure. The licensee expects to begin the cleanup efforts the week of January 5, 1998. Regional Action: The resident inspector responded to the site on December 27, 1997, to review the licensee's actions in response to the event.
