Effective February 3, 1997, Fred Dacimo, formerly Vice President of Nuclear Operations for Northeast Utilities, will be assuming the LaSalle Plant General Manager position. MR Number: 3-97-0007 Clinton 1 Date: 01/28/97 Clinton,Illinois RI PC Dockets: 50-461 BWR/GE-6 Subject: 26,000 GALLONS INADVERTENTLY DRAINED TO SUMP Discussion: On 1/27/97 the licensee inadvertently drained approximately 26,000 gallons of condensate water to the RCIC room sump. The error was caused by personnel error. The first error involved an improperly conducted tag-out procedure. An operator and independent verifier went to the "B" feedwater train rather than the "A" train thereby leaving an "A" train drain valve open when it should have been closed. The licensee had started the condensate system for cleanup purposes; the open drain valve provided the drainage path. In addition to the valve lineup error, control room operators did not properly evaluate an alarm for RCIC sump pump run time. The alarm had come in a number of times over the course of the shift without the operators investigating the source of drainage into the sump. The licensee has initiated a plant wide stand-down to evaluate recent events. Regional Action: The resident inspectors are following up on the performance aspect of this and similar recent valving/tagging errors. Contact: G. C. MR Number: 4-97-0010 Trojan 1 Date: 01/28/97 Ranier,Oregon Dockets: 50-344 PWR/W-4-LP Subject: FIRE IN ADMINISTRTATIVE BUILDING - UPDATE Discussion: Additional information has been provided by the licensee over the past 24 hours concerning the reported fire in the administrative building at the Trojan Nuclear site. This event was originally reported on Friday, January 24, 1997. During investigation of the telephone switch gear cabinets, several large capacitors were found with indications of overheating. These capacitors apparently failed, resulting in the reported smoke and subsequent activation of the halon system. Regional Action: No further action by the NRC is planned. Contact: D.
