After attempts to close the valve were unsuccessful, the licensee decided to shut down the unit to make repairs. At 70 percent power, the licensee inserted a manual reactor scram and tripped the main turbine. Shutdown cooling was entered on May 25, at 8:00 p.m. While lining up for shutdown cooling, the residual heat removal heat exchanger outlet valve would not open from the control room and was manually opened. The outlet valve failure was due to the failure of the torque switch roll pin [see NRC information notice 94-49]. The licensee is evaluating other motor operated valves for the condition. The cause of the turbine control valve problem the failure of the push rod spring housing coupling bolts. Four of the 12 coupling bolts were an incorrect size which may have contributed to the failure. The licensee is awaiting the results of metallurgical analysis to determine the exact failure mechanism. The licensee has replaced all the applicable bolts on the control valves and combined intermediate valves and is in the process of completing work scheduled on their short forced outage schedule. Regional Action: The resident inspectors responded to the site and observed the plant shutdown and are continuing their review. Effective July 1, 1997, Mr. Bill McCollum (currently Catawba Site Vice President) will become the Oconee Site Vice President, taking the place of the retiring Mr. Jim Hampton. In turn, Mr. Gary Peterson (currently Catawba Station Manager) will assume the position of Catawba Vice President. Additionally, Mr. Tony McConnell has accepted a postiion with Duke Engineering and Services, leaving his current position of Station Support Manager in the Nuclear General Office to be filled by Mr. Joe Davis (currently Oconee Engineering Manager). MR Number: 3-97-0073 Braidwood 1 Date: 05/27/97 Braceville,Illinois SRI/RI VIA PC Dockets: 50-456 PWR/W-4-LP Subject: STARTUP FOLLOWING PLANNED OUTAGE Discussion: ON MAY 5, 1997, THE UNIT ONE REACTOR WAS MADE CRITICAL AT 4:44 P.M. (CDT) AND AS OF 7:00 A.M. ON MAY 27 WAS AT 20 PERCENT POWER. THE UNIT WAS SYNCHRONIZED TO THE GRID AT 5:52 A.M. ON MAY 27. THE REACTOR WAS SHUTDOWN ON MARCH 29, 1997, FOR A PLANNED REFUELING OUTAGE. DURING THE OUTAGE THE FOLLOWING MAJOR WORK WAS PERFORMED: MAIN GENERATOR VIBRATION REPAIR, B&C LP TURBINE INSPECTIONS, STEAM GENERATOR TESTING AND REPAIRS, C COLD LOOP STOP VALVE REPAIRS, AND 10-YEAR IN SERVICE INSPECTIONS. Regional Action: THE STARTUP AND POWER ASCENSION WERE MONITORED BY THE RESIDENT INSPECTORS. THE STEAM GENERATOR TUBE INSPECTIONS AND REPAIRS WERE MONITORED BY A REGIONAL SPECIALIST.
