MR Number: 4-97-0008 South Texas 1 Date: 01/27/97 Wadsworth,Texas Resident Inspectors Dockets: 50-498 PWR/W-4-LP Subject: STP UNIT 1 SHUTDOWN GREATER THAN 24 HOURS Discussion: On January 24, 1997, at 7 p.m., licensed operators began reducing reactor power in Unit 1 for a planned 18-hour outage to test reactor control cluster assemblies (RCCAs). Rod testing was completed by 5 a.m. on January 25, as scheduled. The test resulted in two RCCAs sticking at six steps withdrawn. Although this met the licensee's previously established immediate restart criteria, both RCCAs, K-8 in Control Bank A and C-9 in Shutdown Bank B, were in fuel assemblies that were below the core residence time expected to cause the sticking phenomena. The licensee discussed the test results and restart plans with NRR staff over the weekend. Unit 1 was restarted and is at 43 percent power, awaiting calometric results. Licensee engineers are required to fully evaluate these results within 1 week. During the shutdown, reactor plant operators identified four leaking handways on the secondary side of two steam generators. The reactor remained shutdown for an additional 32 hours to perform repairs. The main generator output breaker was closed at 1:45 a.m. on January 27 ending the outage. MR Number: 4-97-0009 Trojan 1 Date: 01/24/97 Ranier,Oregon Dockets: 50-344 PWR/W-4-LP Subject: FIRE IN ADMINISTRATIVE BUILDING Reportable Event Number: 31649 Discussion: Notification was received on January 24, 1997, reporting a small electrical fire in an administrative building at the Trojan Nuclear Site. The effected administrative building is located outside the protected area and outside the industrial security area. The building is typically unoccupied. The fire did not threaten any portion of the reactor facility which is currently shutdown and undergoing decommissioning. The fire occurred at 8:14 a.m. (PST) in the telecommunications room of the administrative building. Local alarms were heard by the Trojan Security personnel, who then notified the fire department. The Rainier Fire Department responded to the fire; however, the fire was extinguished by the Halon system prior to fire department arrival. Smoke was cleared from the room by 9:38 a.m. (PST) by the Rainer Fire Department. Upon investigation of the incident, there appears to be no evidence of fire damage. The reported smoke may have been the halon gas. This incident appears to be a malfunction and activation of the halon system which then activated the local fire alarm. The Oregon Department of Energy was notified. An NRC Resident inspector is not assigned to Trojan. Regional Action: No further action by NRC is planned. Contact: D.
