The rail car is being returned to Shapiro Brothers under a DOT-E exemption UT-MO-97-02 and is expected to arrive in Festus, MO in three to four days. The State of Missouri will perform radiological surveys of the returned shipment to determine the source of radiation when it arrives. MR Number: 3-97-0054 La Salle 1 2 Date: 03/17/97 Marseilles,Illinois SRI VIA TELCON Dockets: 50-373,50-374 BWR/GE-5,BWR/GE-5 Subject: OPERATING CREW FAILURES DURING SIMULATOR EXAMINATIONS (UPDATE) Discussion: During the week of March 17, 1997, LaSalle commenced performance evaluations of remaining operating crews as a result of operator performance deficiencies identified by the licensee during a mid-term training performance evaluation of one operating crew. (See previous Morning Report 3-97-0046.) The licensee has completed performance evaluations on two additional operating crews. A regional NRC operator licensing examiner observed the performance evaluations on one of these operating crews during which the licensee identified that six individuals demonstrated significant performance weaknesses. The licensee rated two individuals from the other crew as unsatisfactory. The licensee indicated that it will consider termination of three individual licenses from these crews. All three of the six crews examined thus far have been rated unsatisfactory by the licensee. After remediation training, the crews were re-examined and evaluated as satisfactory by the licensee. The licensee plans to complete performance evaluations on the remaining three crews within the next two weeks. Operator performance deficiencies observed by the regional NRC operator licensing examiner involved the inability to perform normal and abnormal plant evolutions. In addition, the licensee commenced performance evaluations of their non- licensed operators. Of eight non-licensed operators examined thus far, two passed, two were scheduled for short-term remediation, and four were removed from shift duties until they undergo long term remediation and successful re- evaluation. Both LaSalle units have been shut down since September 1996. Unit 2 remains in a defueled condition.
