MR Number: 1-97-0059 Haddam Neck 1 Date: 11/24/97 Hartford,Connecticut SRI - TELEPHONIC Dockets: 50-213 PWR/W-4-LP Subject: DECONTAMINATION ARTIFACT REMOVAL Discussion: With the plant permanently shutdown for decommissioning and the reactor defueled, the licensee successfully controlled radiological hazards for work completed on November 22 and 23 to remove a 6 foot section of pipe from the reactor. The work included installing a freeze seal, draining and cutting a 3 inch diameter section of letdown line that had contact exterior dose rates of 1000 - 1200 mRem/hr and internal contamination of 40 Rad/hr. Pipe specimens will be sent to a vendor for testing decontamination methods and techniques that may be used for eventual reactor coolant system decontamination. As of 8:00 a.m.on November 24, licensee work activities were in progress to install a replacement spool piece to restore the integrity of the letdown line. Post welding hydrostatic testing is scheduled to be finished by November 25. Regional Action: A Region I Senior Radiation Specialist and the SRI provided round the clock coverage to review the conduct of the work and the adequacy of the worker radiological protection. Region I will follow the licensee's activities through the completion of the new pipe installation and hydrostatic testing. This information is current as of 8:00 a.m.on November 24. Effective November 19, 1997, Guy Horn, Senior Vice President of Energy Supply, has been temporarily reassigned to CNS. He is to develop and implement an integrated strategic plan to address concerns pertaining to the apparent lack of progress in improving both engineering performance and the effectiveness of corrective actions. These concerns mirror those expressed by the NPPD board, the NRC, and the Cooper Independent Assessment Group. He will be assigned to the site long enough to demonstrate improvement and establish a sustainable positive trend in the engineering and corrective action program, estimated to be at least until the SALP assessment is completed in July of 1998. The reporting arrangement for the non-nuclear subordinates will be redistributed to relieve Mr. Horn of day-to-day business responsibilities. In a separate management change, the Design Engineering Manager resigned on November 19. No replacement for this position has been announced. Regional Action: None.
