The proposed notice, "Integrity of Operator Licensing Examinations", will discuss experiences related to onsite and offsite control of the security of examination materials and to control of physical access to the area in which the examination is being given. MR Number: 2-97-0091 Harris 1 Date: 12/24/97 Raleigh,North Carolina Dockets: 50-400 PWR/W-3-LP Subject: WATER HAMMER EVENT AT HARRIS Discussion: On December 22, 1997, at approximately 2:00 p.m. while attempting to initiate Steam Generator (S/G) Blowdown on the C' S/G, a water hammer event occurred following the opening of the outside containment isolation valve. The water hammer event caused a crack in the socket weld on a valve in a two inch branch line off the blowdown pipe (6 inch line) in the Reactor Auxiliary Building. An auxiliary operator was stationed in the auxiliary building per procedure and he notified the control room of the event and the steam leak was isolated. No one was injured. The licensee formed an event review team and performed an evaluation and walkdowns of the piping, equipment and supports. Eighteen supports and 4 mechanical snubbers were affected. A weld repair to the valve was completed one snubber was replaced, and some hangers were repaired. The licensee is completing their evaluation and plans to place the S/G Blowdown system back in service today using a revised procedure. Regional Action: The acting resident inspector was present and followed the licensee corrective actions and inspected the piping and repairs.
