Headquarters Daily Report JANUARY 23, 1997 *************************************************************************** REPORT NEGATIVE NO INPUT ATTACHED INPUT RECEIVED RECEIVED HEADQUARTERS û REGION I û REGION II û REGION III û REGION IV û PRIORITY ATTENTION REQUIRED MORNING REPORT - REGION I JANUARY 23, 1997 Licensee/Facility: Notification: New York Power Authority MR Number: 1-97-0005 Fitz Patrick 1 Date: 01/23/97 Lycoming,New York RI PC Dockets: 50-333 BWR/GE-4 Subject: STEAM LEAK REQUIRING TURBINE BUILDING EVACUATION Discussion: At 4:00 P.M. on January 22, 1997, while attempting to isolate a leaking gauge glass on the sixth stage feed water heater the control room directed a precautionary evacuation of the turbine building when the steam leak increased in size. The plant was operating at 100 percent power and was reduced to 90 percent in anticipation of removal of the feedwater heater from service. The operators subsequently isolated the leak utilizing additional isolation valves and stopped the steam leak. The unit was returned to 100 percent power at 5:00 P.M. The steam leak occurred following shift turnover and had minimal impact on plant operations as the majority of the work force had left for the day. There were no personnel contaminations and the air sample in the area of the feedwater heater indicated no airborne contamination. The heat source for the sixth stage feedwater heater is extraction steam from the high pressure turbine and is at approximately 300 pounds per square inch pressure. The two section gauge glass is connected by 3/4 inch steel piping to the heater shell and provides local indication of the water level. Preliminary investigation by the licensee determined that the gasket separating the upper and lower section of the gauge glass was the source of the steam leak. The leak remains isolated and the repair is being planned for an outage. Regional Action: Resident inspectors are continuing their review of the response to the event. No significant equipment anomalies were noted while placing the plant in hot shutdown. Previously, on November 13, 1996, the licensee reduced power to 88 percent power to isolate the 31B and 32B feedwater heaters due to suspected tube leaks. After reviewing the acceptably of operating the plant at a higher power, the licensee increased power to 97 percent on December 16. On January 18, 1997, the control room received high level alarms within the 31C low pressure feedwater heater. The licensee decided to shut down the plant to inspect and repair the feedwater heaters. The licensee is investigating the cause for the low pressure feedwater heater tube leaks. On January 20, 1997, the licensee identified a leaking pressurizer manway and decided to take the plant to the cold shutdown condition for repairs. Also, several other maintenance and surveillance activities are planned during the outage, including the replacement of the pressurizer power operated relief valves. Although the scope has not been finalized, the forced outage is expected to be from two to three weeks in duration. Regional Action: The resident inspectors are monitoring the licensee's activities. All changes will be effective on February 3, 1997. Mr. Douglas Antony, President of NSP Generation, will retire. Mr. Edward Watzel, currently Vice President, Nuclear, will become the President of NSP Generation. Mr. Michael Wadley, currently the Prairie Island Plant Manager, will become the Vice President, Nuclear. Mr. Joel Sorenson, currently the General Superintendent of Plant Operations, will become the Prairie Island Plant Manager. No replacement for Mr. Sorenson has been named yet.
