Gore And Associates Date: 09/23/97 Cherry Hill,Maryland Subject: GAUGING DEVICE SENT TO LANDFILL Discussion: On September 19, 1997, Region II was informed that a gauging device from W.L. Gore and Associates' Cherry Hill, Maryland, manufacturing facility (State of Maryland licensee), was accidentally sent to a commercial landfill in Charles City, Virginia. The gauge was a Data Measurement Corporation Camtron II containing approximately 860 millicuries of Krypton 85. The incident occurred when the gauge was removed from a production line and placed in a storage area awaiting disposition. A licensee representative incorrectly assumed that the source and its housing were removed from the remainder of the gauging device when the gauge was scrapped. The gauge device and source was placed in the dumpster the last week of August and removed from the site on September 8. On September 17, the licensee discovered that the gauge was accidentally scrapped. The State of Maryland was notified the next day. The gauge emits low energy beta radiation and would not likely result in any significant exposure to members of the public if ruptured due to the rapid dispersion of the radioactive gas in the atmosphere. MR Number: 2-97-0074 Saint Lucie 2 Date: 09/23/97 Florida Dockets: 50-389 PWR/CE Subject: SMOKE DETECTOR FIRE ALARM IN THE CABLE SPREADING ROOM Discussion: At approximately 10:00 p.m. Friday, September 19, 1997, the Unit 2 Control Room received a smoke detector fire alarm in the cable spreading room. The Assistant Nuclear Plant Supervisor (ANPS) responded to the area to investigate. Upon entering the cable spreading room, the ANPS noted the smell of smoke and could see flames coming from the 120-volt AC static uninterruptible power supply (SUPS) circuit panel. At about the same time, the Control Room received an annunciator for a SUPS problem. There was essentially no rise in ambient temperature in the cable spreading room which would be required to actuate the sprinkler system. The ANPS called for the fire brigade, and commenced fighting the fire with a CO2 fire extinguisher. The fire was extinguished within 10 minutes of receiving the alarm and before the fire brigade arrived on the scene. Although the SUPS inverter power was not interrupted, the operators entered the off-normal operating procedure (ONOP) for the loss of the SUPS inverter due to meeting the entry conditions. Per the ONOP the ANPS attempted to transfer power from the normal to the alternate source. When the transfer did not occur the ANPS observed the alternate source transformer, and although the fire was out, the alternate source transformer still appeared to be energized and smoking. The breaker was then opened to deenergize the alternate source transformer and the smoking stopped. Security was requested to open the doors for ventilation and post a guard as required. Maintenance was notified and a condition report was written to investigate the root cause. The initial licensee investigation revealed that the alternate power supply voltage regulator transformer had failed, possibly due to age. This was a 22 year old paper wound transformer that is no longer available from the vendor. The SUPS power panel is a non-safety related uninterruptable power supply for balance of plant loads, including control power for the feedwater regulating valves, feedwater heater drain valves, and main generator control circuitry. At no time were these loads deenergized. The normal power supply and the battery backup power supplies were always functional. Only the third source of power, the alternate power supply, which was the source of the fire was affected. Regional Action: The Resident Inspectors will continue to follow up on the event. MR Number: 3-97-0099 Quad Cities 1 2 Date: 09/19/97 Cordova,Illinois VIA RESIDENT INSPECTOR Dockets: 50-254,50-265 BWR/GE-3,BWR/GE-3 Subject: WEAPON FOUND DURING VEHICLE ENTRY SEARCH Discussion: On September 18, 1997, at approximately 1:55 p.m. a security officer found a loaded .22 Caliber pistol under a mattress in the rear cab of an over-the-road truck. The weapon belonged to the driver. The driver and vehicle were denied access. Licensee interview of the driver determined no malevolent intent.
