Hodge NEW ISSUE. The vendor reports that some 3/4-inch Schedule 40 piping may have smaller wall thickness than the minimum allowed by SA 312 specifications. MR Number: 4-97-0082 Wolf Creek 1 Date: 10/23/97 Burlington,Kansas Resident Inspector Dockets: 50-482 PWR/W-4-LP Subject: INDIVIDUAL PARTIAL FALL INTO TRANSFER CANAL Discussion: On October 22, 1997, at 5:39 p.m. an individual involved with decontamination of the refueling machine inside containment slipped into the transfer canal. The individual's arms checked the fall, immersing the legs to just above the knees. The individual wore the required safety and radiation protection clothing and equipment. The individual suffered a bruise/abrasion to a shin during the fall but was able to walk out of containment. Outside containment, health physics personnel removed the protective clothing and determined there were about 400 counts per minute above background on the individuals shoes, socks, and modesty clothing below the knees. The contaminated clothing was removed and a compression bandage was used to treat the wound. The individual was decontaminated and escorted to access control. The individual was hand frisked by health physics personnel and then passed through the personnel contamination monitor on the first attempt. Site medical personnel treated the injury and released the individual. The individual was then given a whole body count and submitted a urine sample for tritium analysis. The whole body count showed no detectable radioactivity attributable to reactor operations and the tritium analyses are not yet complete. A Region IV health physics inspector was in the radiologically controlled area when the event occurred and responded to inspect the licensee's response. The licensee has initiated an investigation into the root cause of this event. Regional Action: Regional and resident inspectors will continue to monitor the licensee's investigation.
