MR Number: 4-97-0042 Arkansas Nuclear 2 Date: 05/22/97 Russelville,Arkansas RIs Dockets: 50-368 PWR/CE Subject: FUEL BUNDLE MISPLACED IN THE SPENT FUEL POOL Discussion: On May 18, 1997, at 8:12 p.m., while off-loading fuel from the reactor vessel to the spent fuel pool, fuel movement was halted when the licensee discovered that a fuel assembly had been placed, by procedure, in a location in the spent fuel pool which was not allowed by Technical Specifications. Technical Specifications restrict fuel location in the spent fuel pool based on enrichment due to potential critically concerns under boron-free coolant conditions. The licensee determined that an error made in calculating the fuel assembly burn-up resulted in the incorrect determination of remaining enrichment and misclassification of 42 fuel assemblies. As a result, 14 fuel assemblies were designated by procedure to be placed in spent fuel pool locations prohibited by Technical Specifications. The assembly positioned in the incorrect location was the first one removed from the reactor vessel and placed in the pool, but the error was not discovered until an additional seven assemblies had been moved to the spent fuel pool. Upon discovering that one assembly was in the wrong location, the core off-load was halted and the mispositioned spent fuel assembly was relocated to an appropriate location. The licensee performed calculations to determine proper storage locations for the fuel assemblies, corrected the instructions to reflect the proper storage locations, and recommenced the core off-load at 3:54 a.m. on May 19, 1997. The licensee determined that the fuel assemblies in the spent fuel pool remained in a subcritical array with the one fuel assembly in the wrong location. With the existing boron concentrations in the spent fuel pool there was no potential for criticality even if all the improperly classified assemblies had been incorrectly positioned. Kennedy (501)968-3290 _ REGION IV MORNING REPORT PAGE 2 MAY 22, 1997 Licensee/Facility: Notification: Nebraska Public Power District MR Number: 4-97-0043 Cooper 1 Date: 05/22/97 Brownville,Nebraska RIs Dockets: 50-298 BWR/GE-4 Subject: REACTOR STARTUP FROM REFUELING OUTAGE Discussion: On May 20, 1997, at 9:56 a.m., Cooper Nuclear Station began a reactor startup from a refueling outage. The reactor was critical at 1:24 p.m. the same day. Power ascension continued through May 21 and the turbine was synchronized to the grid at 12:47 a.m. on May 22. As of 7:30 a.m. on May 22, the reactor was at approximately 22 percent power. The reactor shutdown to begin the outage was performed on March 28, 1997. Major activities performed during the outage included refueling, installation of a new design of Emergency Core Cooling System torus suction strainers, and modification and refurbishment of all eight safety-relief valve pilot valves.
