MR Number: 3-97-0080 Point Beach 1 2 Date: / / Two Rivers,Wisconsin N/A Dockets: 50-266,50-301 PWR/W-2-LP,PWR/W-2-LP Subject: CLOSURE OF CONFIRMATORY ACTION LETTER (CAL) RIII - 96-017 IN PREPARATION FOR RESTART OF POINT BEACH UNIT 1 & 2 Discussion: Point Beach Unit 2 shut down in September 1996 for refueling and to replace its steam generators and has remained shut down to address a number of significant performance issues described or referenced in CAL RIII-96-017, issued on January 3, 1997. Unit 1 was voluntarily shut down by Point Beach management in February and has remained shut down to better focus resources on addressing the performance issues and restarting Unit 2. Based on Point Beach's satisfactory progress on a large number of improvement initiatives, on July 18, 1997, the Region III Regional Administrator issued a letter to Wisconsin Electric Company closing the CAL. Also, the necessary license amendments to support steam generator replacement on Unit 2 were issued earlier this month. The NRC has no further holds on Unit 2 restart. Point Beach is currently preparing to restart Unit 2. Unit 1 is expected to restart after Unit 2. Regional Action: Region III staff and the resident inspectors are closely monitoring Point Beach's restart efforts. Contact: J. MR Number: 3-97-0081 Monticello 1 Date: 07/22/97 Monticello,Minnesota INFORMATION ONLY Dockets: 50-263 BWR/GE-3 Subject: DELAYED STARTUP DUE TO NEW ECCS SUCTION STRAINER CONCERN Discussion: On May 9, 1997, the licensee commenced a reactor shutdown to facilitate replacement of the ECCS suction strainers due to net positive suction head concerns. The new strainers consisted of several alternating 40 inch and 26 inch diameter subassemblies. Each subassembly was about 4 inches wide and was constructed of perforated (1/8-inch holes) stainless steel. On July 11, the licensee completed the installation phase of the project. During a final walkdown of the torus prior to refilling, the licensee identified rust on a portion of the 4 inch band around a 40 inch diameter subassembly. Further inspection found two additional subassemblies (26 inch diameter) with rust. The licensee determined that the three subassembly bands were fabricated from A-36 carbon steel, not stainless steel as designed. Material receipt documentation shows that all of the perforated sheets used in this project had the same heat number and were also used to construct the Dresden Unit 3 strainers. The licensee and strainer manufacturer do not know how the incorrect sheets (carbon steel) became mixed in with those made of stainless steel. The issue is being reviewed for 10 CFR Part 21 applicability. A reasonable corrosion rate of A-36 could be assumed at 0.001 inch/year; however, this may not be the maximum corrosion experienced. The perforated holes are 1/8 inch in diameter with apparently no specified tolerance band. The diameter of the torus/drywell spray nozzles is 9/64 inch; the core spray nozzle size has not been confirmed. Because of these uncertainties and the desire not to drain the torus during the next two refueling outages, the licensee has decided to extend the current forced outage by 5-7 days to replace the carbon steel pieces. Regional Action: The resident inspectors will continue to observe replacement and testing of the suction strainers. Contact: J. Date: 07/22/97 San Onofre 3 LICENSEE NOTIFIED SENIOR RESIDENT San Clemente,California Dockets: 50-362 PWR/CE Subject: UNIT 3 RESTART FOLLOWING REFUELING OUTAGE NUMBER R9 Discussion: At 2:42 a.m. (PDT), on July 21, 1997, the licensee completed the eighth refueling outage for Unit 3, when the main turbine generator was connected to the grid. The outage began on April 12, and continued for 100 days. Major work included: (1) steam generator eddy current testing and subsequent tube plugging; (2) chemical cleaning of the steam generators, including evaluation of egg crate supports; and (3) repair of inconel 600 nozzels. The licensee plans power ascension testing at a plateau of 80 percent power and anticipates achieving full power on July 22, 1997.
