Hodge NEW ISSUE. Commonwealth Edison, the licensee for Byron, Braidwood, and Zion, reports finding several Vitec transformers in safety-related circuit boards with cracked cores. Temperature changes in these circuit boards could cause associated instrumentation to drift out of range. Petrone NEW ISSUE. Indiana/Michigan Power, the licensee for D. C. Cook, reports that in a lot of seven torque switch assemblies received from Limitorque for SMB-00 actuators, all were missing spacers. This allows rotation of the torque switch shaft and opening of contacts, preventing the actuator from fully closing the valve. Carter NEW ISSUE. Whiting reports a concern with the continued operation of crane 10044. An analysis of the auxiliary hoist on a crane similar to crane 10044 has revealed some overstressed conditions. The overstressed points are not likely to result in failure. The main hoist is not affected by this condition and can be used without restriction. Whiting recommends taking the auxiliary hoist out of service or derating the capacity of the auxiliary hoist to 40 percent of the original rated capacity. Several nuclear power plants may be affected. Fields NEW ISSUE. Wisconsin Electric, the licensee for Pt. Beach, notified the vendor on 03/03/97 of generator bearing failures on an emergency diesel generator because of electric currents in the generator shaft. The vendor reports that design of vendor-modified bearing electrical insulation did not include provisions to insulate the bearing spacer ring. This effectively defeats the bearing insulation and can result in generator bearing failure. _ REGION II MORNING REPORT PAGE 5 MARCH 20, 1997 Licensee/Facility: Notification: Abbott Labs MR Number: 2-97-0017 Vega Alta Irradiator Facility Date: 03/20/97 Vega Alta,Puerto Rico License No: 52-24994-01 Subject: UNQUALIFIED IRRADIATOR OPERATOR Reportable Event Number: 31971 Discussion: On March 19, 1997, the licensee reported that the irradiator at Vega Alta, Puerto Rico had been operated by an unqualified individual for three hours on February 12, 1997. Region II contacted the licensee and found that at approximately 3:00 a.m. on February 12, 1997, a qualified operator became sick and had to leave the facility. The operator left an operator-in-training in charge believing he was fully qualified per the licensee's procedures to operate the irradiator. The licensee indicated that the operator-in-training had not been tested as required but the individual had demostrated full knowledge regarding the operation of the irradiator and the licensee's operating and emergency procedures. The licensee further indicated that no incidents occurred during the three hours in question and that all pertinent personnel has been re-trained in the requirement that a fully certified operator must be present while the irradiator is in operation. MR Number: 3-97-0049 Big Rock Point 1 Date: 03/20/97 Charlevoix,Michigan Dockets: 50-155 BWR/GE-1 Subject: BIG ROCK POINT REORGANIZATION AND DECOMMISSIONING DATE Discussion: A reorganization of the Big Rock Point staff was announced today during an all employee meeting. The changes listed below will be effective March 31 and are aimed at improving the licensee's performance in the areas of radiation protection and operational support. This organizational change will also serve to position Big Rock Point staff for permanent shutdown and decommissioning activities. The current planned shutdown date for Big Rock Point is May 31, 2000. However, the licensee is evaluating the viability of continued plant operation and is expected to reach a decision in the next several weeks. This decision may effect the planned shutdown date for Big Rock Point.
