Date: 03/18/97 Cleveland,Ohio TELECON TO RIII OFFICE, 3-18-1997 Dockets: 03016055 License No: 34-19089-01 Subject: GROUND WATER ENTERS CONTAMINATED BASEMENT AFTER PUMP FAILS Discussion: During the weekend of March 15-16, 1997, a water pump in a manhole at Advanced Medical Systems, Inc.'s (AMS) Cleveland, Ohio facility failed. The pump is connected to the facility's under drain system, and is used to pump ground water from the under drain system and manhole to water storage tanks located inside the facility. After the failure of the pump, approximately 1000 gallons of ground water entered the facility's basement through cracks in the walls. The basement is contaminated with cobalt-60. On March 17-18, 1997, AMS workers vacuumed up the water and transferred it to a storage tank located in a secured, restricted area. In addition, the workers replaced the pump, and installed an alarming water level switch which is tied to the facility's alarm system. The workers measured the cobalt-60 concentration in a sample from the storage tank and obtained a result of 1.3 microcuries per liter. This water will be stored for eventual processing and disposal. AMS workers also tested a water sample from the underdrain system, and no cobalt-60 was detected. Regional Action: Region III will continue to monitor the event. These individual will report directly to the Site Vice President - Palisades. These changes were made to align with management changes announced on March 3, 1997, in which Robert A. Fenech was named Senior Vice President of Nuclear, Fossil and Hydro Operations and Thomas J.
