Date: 06/19/97 San Onofre 3 Licensee notifed SRI San Clemente,California Dockets: 50-362 PWR/CE Subject: UNIT 3 CONTROL ELEMENT ASSEMBLY NOT ATTACHED TO EXTENSION SHAFT Discussion: San Onofre, Unit 3 has been shut down in refueling since April 12, 1997. During preparations for restart, following Refueling Outage R9, the licensee identified that Control Element Assembly (CEA) 91 did not provide the expected results during rod drop testing. During subsequent testing there appeared to be no speed dampening and no change in adjacent source range nuclear instrument detector counts. On June 18, 1997, the licensee concluded that the CEA was not attached to the extension shaft. The licensee determined that they would have to secure from startup operations, cool down, and remove the reactor vessel head to investigate and correct the problem. During the refueling, on April 19, 1997, the licensee had problems with removal of the CEA in location 91, which is one of four four-fingered CEAs that are each inserted into two assemblies. For refueling, the four-fingered CEAs are withdrawn and pinned to the upper guide structure for removal from the vessel; however, during withdrawal CEA 91 became bound in guides located on the extension shaft housing. The licensee subsequently freed the CEA, replaced the old CEA and the extension shaft, and completed the refueling. On June 19, 1997, the licensee issued a press release which stated that resolution of this problem is expected to delay restart of Unit 3 by approximately two weeks. Regional Action: The resident inspectors will monitor the licensee's investigation, including activities to cool down and remove the reactor vessel head.
