Date: 03/17/97 Baltimore,Maryland License No: MD 07144-01 Subject: STOLEN GAUGE IN MD (AGREEMENT STATE LICENSEE) Discussion: On March 17, 1997, officials from the Maryland Department of the Environment's (MDE) Radiological Health Program (RHP) contacted RI to report information about a stolen Troxler moisture density gauge, model 3401B (SN18268), containing 8 mCi Cs-137 and 40 mCi Am-241. The gauge was being used by a State licensee at a field site near the intersection of Routes 175 and 32, Odenton, Maryland (Anne Arundel County) when it was discovered missing on the morning of March 13, 1997. The licensee, EBA Engineering, Inc., Baltimore, MD, notified MDE that afternoon. First responders to the area on March 13, 1997, included the Anne Arundel County Police Department, their Emergency Response Unit, and a county helicopter flyover. A press release was issued by Anne Arundel County. MDE dispatched an RHP inspector to the scene on March 14, 1997 to perform radiation surveys and interview licensee personnel. An event summary and possible enforcement action is under development by RHP staff. The gauge remains missing. Regional Action: Will continue to monitor. MR Number: 3-97-0046 La Salle 1 2 Date: 03/17/97 Marseilles,Illinois SRI VIA TELCON Dockets: 50-373,50-374 BWR/GE-5,BWR/GE-5 Subject: OPERATING CREW FAILURES DURING SIMULATOR EXAMINATIONS Discussion: During the week of March 10, 1997, as part of LaSalle's ongoing readiness training program, the licensee conducted mid-term performance evaluations for operating crews on the simulator. This was the first week of mid-term evaluations. The scenarios were designed to test operator response to events involving the use of abnormal operating procedures (AOPs). The operating crew evaluated was split into Unit 1 and Unit 2 crews and both crews failed the scenarios. In addition, the licensee identified that three individuals demonstrated significant weaknesses. The licensee stated that the primary reason for the failures was that the operating crews and individuals did not meet performance standards for "command and control" and "procedure adherence" and that the failures did not indicate nuclear safety concerns. The licensee's Performance Review Committee met on March 14, 1997, to evaluate these deficiencies for generic performance issues. As a result of this meeting, the licensee decided to drop one individual's license, and to conduct remediation training for the other two individuals, as well as the failed crews, before allowing them to resume shift duties. The licensee also suspended the current phase of the readiness training program and instead plans to conduct performance evaluations for the remainder of the operating crews commencing on March 21, 1997. The licensee plans to use the results of these evaluations to further develop licensed operator remediation training plans. Both LaSalle units have been shut down since September 1996. Unit 2 remains in a defueled condition. Regional Action: Regional operator licensing examiners will observe selected licensee performance evaluations of the operating crews and during the week of April 7, 1997, will conduct a review of the licensee's remediation plans.
