Bakken replaces Dave Garchow who will move to Engineering as Director-Design Engineering for the Nuclear Business Unit. Bakken joined PSE and G as Manager-Salem Operations in 1995 from Duquense Light Company where he was a nuclear shift supervisor at Beaver Valley. He has since been promoted to Director- Salem Operations. Garchow is returning to the organization where he began his career with PSE and G as a Director of System Engineering in 1995. Linville (610)337-5129 _ REGION II MORNING REPORT PAGE 2 SEPTEMBER 17, 1997 Licensee/Facility: Notification: Florida Power Corporation MR Number: 2-97-0072 Crystal River 3 Date: 09/17/97 Crystal River,Florida Dockets: 50-302 PWR/B&W-L-LP Subject: CRYSTAL RIVER VACUUM IN MAIN CONDENSER Discussion: On Saturday, September 13, Crystal River Unit 3 admitted auxiliary steam into the plant from the adjacent coal plants (Units 1 & 2) and drew a vacuum in the main condenser. This was achieved two days earlier than planned. The secondary plant has been in lay-up for over a year since Crystal River shut down on September 2, 1996. The evolution went very well and only minor complications were encountered. Chemistry results were also extremely favorable and allowed the transition to long cycle cleanup from short cycle earlier than expected on Saturday September 13. Levels of dissolved solids and corrosion products were very low and oxygen in the condensate is currently only 3.8 ppb. The licensee has run both condensate pumps and a booster pump so far and they plan to run the remaining secondary pumps, including the feed pumps through this week. They plan on maintaining vacuum until this weekend to support numerous post-maintenance tests. Installation of a higher capacity radiator for the "A" EDG is continuing with completion of the modification and testing to restore operability scheduled for this week. The "B" EDG radiator upgrade is scheduled to commence next week. Both EDGs have been modified to increase their load capacity ratings by 150 kW. Testing to verify the increased load rating for both EDGs is scheduled for November. Regional Action: Resident and Regional Inspectors will follow the above activities. Contact: S. Dockets: 03001435 License No: 12-02642-06 Subject: NOTIFICATION OF LOST SOURCE Discussion: On September 15, 1997, Region III was notified that a Beckman Model LS250, Serial 159 liquid scintillation counter (LSC) was shipped to a scrap dealer named General Iron in Chicago, IL on September 13, 1997. The LSC contained a sealed source with a nominal activity of 40 microcuries of Cesium-137 as of 1954 (i.e., about 15 microcuries today). The source was within about 250 pounds of lead. General Iron shredded the device on September 13, 1997. The LSC scrap was likely shipped to a smelter. General Iron's radiation monitors were not tripped by the event. The smelting firm is equipped with radiation monitors. The licensee planned to inform General Iron of the lost source and attempt to perform radiation surveys at General Iron in coordination with the Illinois Department of Nuclear Safety (IDNS). Regional Action: Region III notified the IDNS on September 15, 1997. The event will be reviewed during the next routine inspection. MR Number: 3-97-0097 Big Rock Point 1 Date: 09/17/97 Charlevoix,Michigan INSPECTION Dockets: 50-155 BWR/GE-1 Subject: TWO FUEL BUNDLES STUCK IN THE CORE Discussion: The Big Rock Point reactor was shut down on August 29, 1997, with the reactor core containing 84 fuel bundles. On September 5, 1997, core off-load began. During the core off-load, two fuel bundles, in core locations 10-56 and 06-59, did not move with normal pull force. These two fuel bundles were skipped and core off-load continued. On September 9, 1997, 82 of the 84 fuel bundles had been off-loaded. The fuel bundles became stuck due to warping in both the fuel rods and in the channel that holds the fuel bundles. Other fuel bundles had also been stuck, but pulling with additional force resulted in them moving. On September 11, 1997, with the assistance of the fuel manufacturer, an attempt was made to move one of the stuck fuel bundles by applying extra pulling force. The fuel bundle still failed to move. The fuel manufacturer then manufactured stronger tie rods. When a new stronger tie rod was pull tested, the rod broke and failed the pull test; therefore, the plan to strengthen the fuel bundles to allow using a greater pulling force for removal was canceled. The fuel manufacturer has developed a process which will rebuild the fuel bundles. The fuel pins will be moved one at a time from the fuel bundles into a new fuel skeleton (new grids and upper and lower tie plates). This rebuilding work will be performed in the core. The new rebuilt fuel bundles will then be moved to the spent fuel pool using normal defueling activities, completing the final defueling of the core. The work is expected to start on September 18, 1997, and to last 3 days. Regional Action: The senior resident inspector will follow the work and fuel bundle moves.
