NEW ISSUE.  The Hope Creek licensee reports that thermal degradation of  
the bearing pad material of the Model 219 NE relay has been confirmed    
with the manufacturer, Struthers-Dunn.  This failure mechanism occurs    
when the coil is energized, leading to chattering of the armature and    
contacts.  The relay can fail with open contact, closed contact or       
shorted coil.  This failure mechanism has been observed only for relays  
with alternating current coils.  The degraded material is magnetic vinyl 
plastic.  The proper material is silicon rubber.

NEW ISSUE. On September 16, 1996, the Perry licensee reported a false    
trip of a K600S K-Line low voltage alternating current breaker with a    
SS-5 solid state trip device.  The vendor found crossed phase sensor     
wires resulting in reversed polarity.  The cause was human error during  
1995 breaker manufacture.

Subject: ZION ACTIONS RESULTING FROM REACTIVITY EVENT                    

Discussion:

The licensee informed the resident inspectors that it is currently       
drafting a letter to the NRC requesting that the NRC licenses be dropped 
for the primary reactor operator, unit supervisor, and shift engineer who
were involved in the reactivity excursion event during a Unit 1 shutdown 
on February 21, 1997. In addition, the licensee is taking disciplinary   
action against operations management involved in the event.
