MR Number: 2-97-0046 North Anna 1 Date: 06/16/97 Richmond,Virginia Dockets: 50-338 PWR/W-3-LP Subject: NORTH ANNA UNIT 1 POWER REDUCTION TO REPAIR STEAM LINE GASKET LEAK Discussion: On June 14, 1997, while North Anna Unit 1 was in power ascension from a refueling outage, a steam leak occurred at the six-inch flanged connection where the Moisture Separator Reheater (MSR) vents into the 1A feedwater heater extraction steam line. Power was reduced from 87 percent to 56 percent to facilitate gasket replacement. The licensee replaced the gaskets on both the A and B extraction steam lines. These gaskets had been replaced during the recent refueling outage as part of a MSR replacement project. The unit resumed power ascension on June 15. The licensee is presently evaluating the cause of the gasket failure. There were no personnel injuries or detectable radioactive release. Regional Action: Routine followup Contact: L. GARNER (404)562-4552 _ REGION IV MORNING REPORT PAGE 3 JUNE 16, 1997 Licensee/Facility: Notification: Nebraska Public Power District MR Number: 4-97-0052 Cooper 1 Date: 06/13/97 Brownville,Nebraska Resident Inspector Dockets: 50-298 BWR/GE-4 Subject: APPROVAL OF REQUEST FOR ENFORCEMENT DISCRETION Discussion: On June 14, 1997, Region IV and the Office of Nuclear Reactor Regulation approved a request for regional enforcement discretion from the Nebraska Public Power District for the Cooper Nuclear Station. The NRC approval granted an additional 24 hours to the limiting condition for operation (LCO) associated with Technical Specification (TS) 3.7.A.5, "Oxygen Concentration." The TS 3.7.A.5 LCO required the plant to be in the cold shutdown condition within 24 hours if the containment atmosphere oxygen concentration could not be maintained less than 4 percent by volume. The need for enforcement discretion was identified after the correction of some minor unidentified leakage following the recent startup from a planned refueling outage. On June 14, 1997, reactor power was reduced and the containment de-inerted to allow personnel to identify and terminate the leakage. Following repairs, the containment inerting process was initiated. The licensee found that the inerting procedure required more time to complete than what time remained in the 24-hour LCO. The exercise of enforcement discretion provided the licensee sufficient time to restore the proper containment atmosphere without incurring an unnecessary plant transient and minimized radiation exposures to plant personnel. Regional Action: Region IV will issue a formal letter approving the enforcement discretion as described in NRC Administrative Letter 95-05, "Revisions to Staff Guidance for Implementing NRC Policy on Notices of Enforcement Discretion," and consider the proper enforcement actions.
