The crack may cause a short between the contact and the frame of the polar element. The crack may be caused by the excessive length of the screw that retains the contact block to the polar element frame. According to the current owner of Fife Company (Measurex), the only records of transfer that exist indicate that the gauge was initially transferred to Huntsman Container Corporation, in Fullerton, California in 1978 and is no longer in business. Region III could not identify the current owner(s) of the gauge. Regional Action: Region III notified the EPA and the State of Ohio that the gauge was removed from the scrap yard and transferred to Measurex in Maryland for disposal on 10/10/97. Contact: DARREL WEIDEMAN (630)829-9808 _ REGION IV MORNING REPORT PAGE 9 OCTOBER 15, 1997 Licensee/Facility: Notification: Nebraska Public Power District MR Number: 4-97-0077 Cooper 1 Date: 10/15/97 Brownville,Nebraska SENIOR RESIDENT INSPECTOR TELECON Dockets: 50-298 BWR/GE-4 Subject: 4KV SAFETY-RELATED BREAKERS NOT OVERHAULED Discussion: On October 7, 1997, the licensee identified that six (later determined to be five) General Electric Magna-Blast circuit breakers in 4KV safety-related applications at Cooper had not been overhauled by the breaker manufacturer in approximately 23 years of plant operation. The licensee had been performing preventive maintenance activities on the breakers. These five breakers were the emergency diesel generator output breakers (2), two residual heat removal pump motor breakers, and a service water pump motor breaker. This condition was discovered during followup to a breaker malfunction on October 5 in a nonsafety-related application. That malfunction was attributed to a lubrication problem on a roller bearing surface not accessible during routine preventive maintenance. On October 9, 1997, the licensee completed an operability assessment which concluded that the essential breakers were operable. This conclusion was based primarily on satisfactory breaker operation during routine testing, completion of scheduled preventive maintenance, and an inspection of the breakers performed on October 8, 1997 which found the breakers to be in good condition with no evidence of binding or interferences. On October 14, 1997, the licensee completed a revised operability assessment which addressed the safety impact of 10 additional nonessential breakers which also had not been overhauled. The licensee is currently developing a schedule to perform overhaul of the breakers. Regional Action: Region IV will perform a special inspection beginning the week of October 20, 1997.
