MR Number: 3-97-0103 La Salle 1 2 Date: 10/03/97 Marseilles,Illinois SRI VIA TELEPHONE 10/3/97 Dockets: 50-373,50-374 BWR/GE-5,BWR/GE-5 Subject: INSTITUTE FOR NUCLEAR POWER OPERATORS (INPO) PLACES OPERATOR TRAINING PROGRAM ON PROBATION Discussion: On September 22, 1997, the National Nuclear Accrediting Board placed the following training programs at LaSalle on probation: - nonlicensed operator - reactor operator - senior reactor operator - shift manager - continuing training for licensed personnel - shift technical advisor The accrediting board placed the programs on probation for 180 days. The report noted that the nonlicensed operator in-plant training conducted by operations mentors was of consistently high quality. However, the following three areas were considered to be the most important in need of improvement: - Previously identified actions to correct problems have not been fully implemented and effective. - Feedback from first line supervisors and training managers was not provided to improve the training program by identifying weaknesses and areas where program changes were necessary. - Corrective actions were not taken to address instructor deficiencies due to a lack of accountability and clear priorities. INPO requested that Commonwealth Edison representatives meet with the National Nuclear Accrediting Board in March 1998 after additional INPO review of the corrective actions implemented by Commonwealth Edison. Regional Action: A NRC restart panel has been established to review activities at LaSalle. The panel is using the guidance in NRC Inspection Manual Chapter 0350. Contact: M. MR Number: 4-97-0076 Palo Verde 2 Date: 10/14/97 Wintersburg,Arizona Licensee to SRI Dockets: 50-529 PWR/CE80 Subject: UNIT 2 RESTART FOLLOWING REFUELING OUTAGE 7 Discussion: Palo Verde Unit 2 attached to the grid at 9:49 a.m. (MST) on October 13, 1997, following completion of its 37-day Cycle 7 refueling outage. The outage, completed on schedule, included steam generator eddy current testing and sludge lancing, core barrel removal for the reactor vessel 10-year inservice inspection, high pressure turbine inspection, Emergency Diesel Generators A and B 5-year inspections and replacement of two reactor coolant pump motors. The startup was completed with no problems. Power ascension testing is continuing and the licensee anticipates achieving full power on October 16, 1997.
