MR Number: 1-97-0020 Oyster Creek 1 Date: 03/12/97 Forked River,New Jersey Dockets: 50-219 BWR/GE-2 Subject: SUBMERGENCE FEATURE OF SUPPRESSION POOL (TORUS) Reportable Event Number: 31932 Discussion: On 3/11/97, the licensee identified that they had created a four inch pathway that partially circumvented the water volume of the torus for about 10 minutes. A normally closed 4" valve (V-21-18) in piping that branches from the containment spray system header and discharges to the air space of the torus for suppression pool spray was stroked open for maintenance. The containment spray system had been made inoperable for scheduled on-line maintenance. Concurrently, an adjacent containment spray system header branch, via 6" flow test valve V-21-17, remained open (a normally open valve) during the maintenance. V-21-17 discharges to one of the drywell downcomers. Consequently, with both valves open, some steam, in the event of a LOCA, would enter the containment spray system piping through V-21-17 (downcomer) and V-21-18, and discharge back to the torus air volume, effectively bypassing the torus water volume. Following the discovery, V-21-17 was tagged closed for further maintenance and stroking of V-21-18. Regional Action: Routine resident followup on the associated tagout errors. Contact: Stephen M. Mr. Steve Barrett will be assuming the position of Dresden Station Operations Manager effective immediately. Mr. Barrett has been assigned to Dresden since August 1994 in the positions of Radiation Protection Manager and, most recently, assistant to the Dresden Site Vice- President. Contact: P. L.
