Headquarters Daily Report MAY 13, 1997 *************************************************************************** REPORT NEGATIVE NO INPUT ATTACHED INPUT RECEIVED RECEIVED HEADQUARTERS û REGION I û REGION II û REGION III û REGION IV û PRIORITY ATTENTION REQUIRED MORNING REPORT - REGION II MAY 13, 1997 Licensee/Facility: Notification: Virginia Power Co. MR Number: 2-97-0037 Surry 1 2 Date: 05/08/97 Surry,Virginia Dockets: 50-280,50-281 PWR/W-3-LP,PWR/W-3-LP Subject: LETDOWN LINE ORIFICE DEGRADATION Discussion: This information is provided to highlight a possible generic problem. An examination of Surry Unit 1 letdown line orifices was conducted due to previous degradation problems identified in Surry Unit 2. The deterioration of the letdown orifices in Unit 2 was a contributing factor to the vibration induced socket weld cracks on piping downstream of the orifices. Cracked welds had resulted in online repairs to these welds. The last repair occurred in January 1997. On March 15, 1997, radiographic examination revealed that the Surry Unit 1 letdown line orifices were deteriorated. The 45 gpm orifice, nominal 0.212 inch inside diameter, was eroded to approximately 1 inch inside diameter in the last five inches of the orifice. One 60 gpm orifice had only minor deterioration while the other 60 gpm orifice, nominal 0.242 inch inside diameter, was eroded to approximately a half-inch inside diameter in the last one and one half-inches of the orifice. Unit 1 has no history of cracked welds on the piping downstream of the letdown line orifices. No cracking was detected during the Unit 1 inspection; however, due to the deterioration, the letdown line orifices were replaced. In addition, portions of the downstream piping were replaced with sections containing butt welded joints. Regional Action: The resident staff has reviewed the planned actions to replace the Unit 2 letdown line orifices during the upcoming refueling outage in October 1997. Region II is considering proposing an information notice to alert other licensees of this issue. Contact: G. A. Belisle (404)562-4550 _
